Purpose. In the present study, the three-dimensional power Doppler was used as a quantitative method to evaluate its reliability in detecting and assessing of gestational trophoblastic disease (GTD). Methods. 52 GTD patients who received diagnosis and treatment at the first affiliated hospitals of Xi'an Jiaotong University in China between 2011 and 2013 were evaluated using Voluson E8 (GE Medical System). Demographic information, pathological characteristics, clinical history, sonographic images, and related indices (resistance index, vascularization index, and flow and vascularization index) were evaluated. Result. Three-dimension power Doppler indicated that there were significant differences in the resistance index, vascularization index, flow index, and vascularization-flow index between the healthy individuals and each subgroup of patients (P < 0.01). Further, in combining invasive hydatidiform mole and choriocarcinoma groups, there was a significant difference between hydatidiform mole and the combined malignant group (P < 0.01). And the abnormal sonographic and power Doppler findings in GTD were resolved when chemotherapy was done successfully. Conclusion. Combined with the clinical features, sonography and three-dimension power Doppler imaging were helpful in diagnosing GTD as a noninvasive method, distinguishing the invasive nature of disease, detecting the recurrence of the disease, and assessing the effectiveness of the chemotherapy.

Objective: To analyse the diagnostic performance of endometrial volume calculated by three-dimensional (3D) ultrasound for diagnosing endometrial carcinoma in women with postmenopausal bleeding. Methods: An extensive search of papers analysing the role of endometrial volume calculated by 3D ultrasound for diagnosing endometrial carcinoma in women with postmenopausal bleeding was performed in MEDLINE/PubMed and Web of Science from January 1996 to January 2020. Quality was assessed using the Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2) tool. Results: The extended search identified 318 citations but after exclusions, eight articles wereincluded in the meta-analysis. The risk of bias for most studies was high for the four domains assessed in QUADAS-2. Overall, after excluding three studies that contributed significantly to heterogeneity, pooled estimated sensitivity and specificity for diagnosing endometrial cancer were 87% (95% confidence interval: 80–92%) and 60% (95% confidence interval: 51–68%), respectively. Heterogeneity was low or moderate. Conclusion: Endometrial volume as estimated by 3D ultrasound using virtual organ computer-aided analysis (VOCALTM) software has a moderate diagnostic performance for detecting endometrial malignancy in women with postmenopausal bleeding.

BackgroundUltrasonography has been extensively used in women suspected of having a gynecological malignancy. The aim of this study is to evaluate the efficacy of 3D ultrasonography and power Doppler for discrimination between benign and malignant endometrium in premenopausal women with abnormal uterine bleeding.MethodsThis cross-sectional study included 78 premenopausal women with abnormal uterine bleeding scheduled for hysteroscopy and endometrial curettage. The endometrial thickness (ET), uterine artery pulsatility index (PI) and resistance index (RI), and endometrial volume (EV) and 3D power Doppler vascularization index (VI), flow index (FI), and vascularization flow index (VFI) were measured and compared with hysteroscopic and histopathologic findings.ResultsThe ET (P <0.001), EV (P <0.001), and endometrial VI (P <0.001) and VFI (P = 0.043) were significantly increased in patients with atypical endometrial hyperplasia and endometrial carcinoma (n = 10) than those with benign endometrium (n = 68); whereas, the uterine artery PI and RI and endometrial FI were not significantly different between the two groups. The best marker for discrimination between benign and malignant endometrium was the VI with an area under the ROC curve of 0.88 at a cutoff value of 0.81 %.Conclusion3D ultrasonography and power Doppler, especially endometrial VI, may be useful for discrimination between benign and malignant endometrium in premenopausal women with abnormal uterine bleeding.
