Three‐Dimensional Echocardiography in the Evaluation of Right Ventricular Global and Regional Systolic Function in Patients with Atrial Septal Defect before and after Percutaneous Closure

Abstract: RV global and regional systolic functions are impaired in open and closed ASD. RT3DE-derived systolic function parameters are negatively correlated with RV after load. RT3DE has potential value in the evaluation of RV systolic function in patients with ASD.

“…However, as CMR combines accurate volumetric assessment with accurate flow quantification and is moreover appealing for myocardial strain imaging, it is increasingly used to study the pathologic RV. Literature shows variable, often contradictory, results with regard to global RV systolic performance in ASD patients [22][23][24][25][26]. Our findings-indicative of impaired function-are in line with the work of Stephensen et al [27].…”

“…There was no change in the global RV systolic function but a high basal RV systolic function decreased after closure. Some patients had impaired diastolic function before closure of defect, which reversed to normal within 6 months after closure [ 15 ]. TAPSE is a standard RV parameter that gives quantitative information about RV function.…”

“…In our cohort, there was a significant increase in the LVEDV and GLS 6 months post-ASD closure. In contrast, it was previously published that circumferential strain was increased significantly 24 hours after ASD closure and might be used as an indicator of LV response to normalized loading conditions [ 14 , 15 ]. LVLS was not affected early after ASD closure may be due to later improvement after 6 months after closure [ 13 ].…”

“…Following Starling's law of the heart, the increased preload results in high ventricular ejection function. e parameters of RV function such as RV ejection fraction, TAPSE, S, and RV FAC tend to be elevated in patients with ASD and are decreased after device closure [21][22][23]. e degree of RV ejection fraction depends on RV volume overload.…”
