Three-Dimensional Bone Block Planning for Mandibular Sagittal Bone Defect Reconstruction

Abstract: Bone defects seen in severe sagittal discrepancies between the maxilla and mandible do not solely qualify for orthodontic treatment. An interdisciplinary approach with the aid of a surgical, orthodontic, and periodontal team should be implemented in the treatment of such cases. Despite the use of standard treatment methods, the therapy is always planned and carried out individually for each patient. The surgical treatment of bone defects in the area of the jawbones is associated with a number of potential comp… Show more

“…A latex drain, or Kaltostat, measuring 1.5-2 cm/5 cm, was inserted into the wound, and sutures were placed on top of the mandibular alveolar region and in the release incision. The drain was removed on postoperative day 1, resulting in a higher healing rate and a scar that did not interfere with tissue function [15]. The technique of inserting a drain into the release incision has many advantages.…”

“…Securing the extraction site with sutures reduces postoperative bleeding and prevents PEA (post-extraction alveolitis) by keeping the clot in the alveolus. By maintaining drainage for 24 h, evacuation of inflammatory exudate is expected and, because of faster removal of the drain, healing by rhizotomy, which includes the first 24 h [15]. This study aimed to evaluate the effect of intraoral drainage with flat latex and Kaltostat drains applied to the buccal side on the severity of complications after surgical removal of the lower wisdom tooth, and we present this method as easy, effective, and likely to find widespread use.…”

“…This means that the dehiscence is bilateral. In turn, according to the modification of this classification presented by Dominiak et al, which is more detailed, such a defect could be classified as BDIEII dehiscence from the vestibular side, which reaches an apical foramen, and bone loss from the lingual side is at the level of 1/2 of the root's length [16].…”

“…After milling, the block was cleaned, packed, sterilized, and sent to the surgeon. A detailed description of bone block planning, including diagrams of the procedure, has been previously described [16].…”

“…The length and number of screws depended greatly on the clinical situation, such as recipient bone conditions and the thickness of the block. 87 Improper fixation of the graft may cause tension within forming tissues and eventually constrain bone healing. A thorough surgical procedure strategy and fine management of soft tissues, including adequate flap design, are essential for osseointegration and more predictable wound healing.…”
