Three cases of type-1 complex regional pain syndrome after elective total hip replacement

Abstract: Complex regional pain syndrome (CRPS) constitutes an atypical cause of pain after orthopaedic procedures. To our knowledge, there is a paucity of literature reporting this syndrome after total hip arthroplasty (THR), since only two case reports have been published. We thenceforth describe the clinical outcome of three cases of type-1 CRPS developed after elective THR, two of them initially diagnosed with secondary osteoarthritis whereas the remaining one presented a sequel of a failed osteosynthesis that requi… Show more

“…It was not possible to confirm whether the patients in these articles met one or more of the CRPS criteria sets. However, these patients were diagnosed by the authors as having reflex sympathetic dystrophy, algodystrophy or CRPS (Christensen et al, 1982 ; Dirksen et al, 1987 ; Duncan et al, 1988 ; Dwyer, 1952 ; Glick, 1973 ; Glick & Helal, 1976 ; Grundberg, 1996 ; Kalita et al, 2006 , 2016 ; Kinov, 2001 ; Klein & Klein, 1991 ; Poplawski et al, 1983 ; Russek et al, 1953 ; Steinbrocker et al, 1953 ; Sussman, 1952 ; Tountas & Noguchi, 1991 ; Varitimidis et al, 2011 ; Zanotti et al, 2017 ; Zych‐Litwin & Litwin, 2019 ; Zyluk, 1998 ; Zyluk & Puchalski, 2008 ).…”

“…However, further issues should be considered when handling a painful HRA, mainly the ALVAL reaction with pseudotumor formation secondary to metal ion release and possible femoral neck stress fractures (Su 2020). Although rare, CRPS has been reported to cause unexplained disproportionate pain after total knee and hip arthroplasties (Royeca et al 2019;Zanotti et al 2017;Hossain and Andrew 2010;Söylev and Boya 2016).…”

“…Interestingly, rare extrinsic causes of painful THA have been suggested in the literature, including complex regional pain syndrome (CRPS), which is characterized by pain distal to the site of physical trauma, unproportionate to the procedure performed, and little or no improvement is observed over time (Royeca et al 2019;Zanotti et al 2017;Hossain and Andrew 2010;Söylev and Boya 2016). The current case report describes a rare presentation of CRPS after multiple THA revision surgeries.…”
