PurposeThe present study aimed to examine the age and gender distributions among chronic cough patients referred to a tertiary cough clinic in Korea, and to investigate clinical factors related to the demographic findings.MethodsStudy participants were unselectively recruited from adult chronic cough patients who attended the cough clinic for the first time during one year. To validate their representativeness, their age and gender distributions were compared to the entire chronic cough population, or with those presenting with other chronic disease. Data from the baseline investigations were analyzed to identify clinical factors related to the demographic findings.ResultsA total of 272 chronic cough patients were included. They had a middle-aged female predominant feature (mean age: 52.8±15.7 years and female 69.1%). Their age and gender distributions were almost identical to the entire chronic cough population, but were distinct from patients with hypertension. Among clinical factors, the older female predominance was associated with enhanced capsaicin cough sensitivity, and also with the presence of 'cough by cold air' symptom. Allotussia and laryngeal paresthesia were highly common in chronic cough patients, affecting 94.8% and 86.8% of them, respectively.ConclusionsThe present study demonstrated older female predominance among adult chronic cough patients attending a referral cough clinic in Korea. The demographic features were significantly associated with the capsaicin cough responses and also potentially with allotussia (particularly cold air as the trigger). These findings suggest a role of cough reflex sensitization in the pathophysiology of chronic cough in adults.

Cough is one of the most common symptoms that causes patients to seek outpatient medical care. If cough persists longer than 8 weeks, common causes of chronic cough, such as upper airway cough syndrome, asthma, and gastroesophageal reflux disease (GERD), should be considered. Although not a common cause of chronic cough, achalasia may cause symptoms very similar to reflux that can lead to its misdiagnosis as GERD. In this report, a 40-year-old woman presenting with chronic cough was initially diagnosed with GERD; however, her symptoms were refractory to conventional GERD treatment. Finally, she was diagnosed with achalasia. Her cough improved completely after pneumatic dilatation. Achalasia is a rare disease accompanied by dysphagia or regurgitation. If cough presumably due to GERD does not respond to treatment, or if the cause of chronic cough is uncertain, physicians should suspect achalasia.

Öksürük, havayollarının temizlenmesinde ve korunmasında rol oynayan kompleks bir refleks mekanizmadır. Larinks ve trakeobronşiyal ağaçta, özellikle de karinada ve bronşların dallanma noktalarında bulunan duyu sinirlerinin çeşitli inflamatuvar, mekanik veya kimyasal dürtüler ile uyarılması sonucunda meydana gelmektedir. Hızlı adapte olan reseptörler (RAR), yavaş adapte olan gerilim reseptörleri (SAR) ve C lifleri olmak üzere temelde üç tip reseptör bu refleks mekanizmada rol oynamaktadır. İki aydan uzun süreli öksürükler kronik öksürük olarak adlandırılmakta, etyolojide çok fazla hastalık yer almaktadır. Çevresel irritan maruziyeti ve immunsupresyonu olmayan, sigara içmeyen, akciğer grafisi ve solunum fonksiyon testleri normal olan kişilerde sıklıkla; öksürükle seyreden astım, üst havayolu öksürük sendromu ve gastroözofagiyal reflü hastalığı öksürükten sorumlu tutulmaktadır. İleri yaş, kadın cinsiyet, ACE inhibitörü antihipertansif ilaç ve sigara kullanımı, kronik hastalıklar, viral enfeksiyon, otoimmun hastalıklar, atopi, gastroözofagiyal reflü varlığı öksürük refleks duyarlılığını artırmak-ta, birden fazla faktörün bir arada olması durumunda ise herhangi bir nedenle oluşan öksürük kronikleşmektedir.AnAhtAr sözcükler: Kronik öksürük, öksürük refleksi, öksü-rük hipersensitivite sendromu, idiyopatik öksürük Cough is one of the complex reflex mechanisms which plays role in clearing and protecting of the airways from secretions, irritants, foreign particles etc. It arises from variety of sensory nerves located in main carina primarily and other bronchial branching locations in trachea-bronchial tree and larynx with inflammatory, mechanical or chemical impulses. Rapidly adapting receptors (RAR), slowly adapting stretch receptors (SAR) and C fibers are three types of receptors basically involve reflex mechanisms of cough. A cough that lasts for over eight weeks is called as a "chronic cough" and there are several causes of chronic cough. In such patients who are never smoked, free from exposure to environmental irritants, with normal chest radiograms and pulmonary function tests; coughvariant asthma, upper airway cough syndrome and gastroesophageal reflux disease are the most common causes of cough.
