Thornwaldt cyst is a rare cystic formation, located along the midline of the nasopharynx. We present the case of a 60-year-old man with impaired nasal breathing and a several months-long history of serous otitis media. His only concomitant disease was arterial hypertension. The diagnostic imaging tests revealed a well-rounded cystic formation involving the upper part of the nasopharynx, characteristic of Thornwaldt cyst. Following, endoscopic transnasal marsupialization was performed and the benign cystic nature was confirmed on histopathology. The patient responded to the administered treatment and reported no persistence or emergence of new symptoms. The current case presents a symptomatic Thornwaldt cyst successfully treated by endoscopic transnasal marsupialization.

Se presenta el caso de una mujer de 49 años, que acudió al servicio de otorrinolaringología con tiempo de enfermedad de tres meses presentando dolor y rigidez cervical, además de inestabilidad. Al ser evaluada, en la resonancia magnética de fosa posterior con contraste, se observó una lesión nodular en nasofaringe compatible con quiste de Thornwaldt. La endoscopia nasal evidenció una lesión compatible con dicho diagnóstico. Se decidió tratamiento quirúrgico, realizándose marsupialización bajo abordaje endoscópico transnasal. En el postoperatorio, la paciente refirió mejoría del dolor y de la rigidez cervical, pero persistió la inestabilidad. Por tratarse de una patología infrecuente y cuya forma de presentación es poco común se reporta el caso y se realiza una revisión del tema.

ÖZETNazofarengeal bursa olarak da bilinen Thornwaldt bursası, nazofarenks orta hattında bulunan bir çıkmaz olup, persistan notokord kalıntılarından meydana gelmiştir. Farengeal bursa, nazofarinks posterior duvarı boyunca longus kapitis adelesi arasında uzanan solunum epiteli ile kaplı bir boşluktur. Bursa ağzı tıkandığından Thornwaldt kisti olarak bilinen benign mukozal nazofarenks kisti gelişir. Sık görülm-esine rağmen, nazofarenksi tamamen doldurup tıkanıklığa yol açması enderdir. Çoğu zaman asemptomatiktirler. Bununla birlikte enfekte olduklarında veya travmaya maruz kaldıklarında halitozis, oksipital baş ağrısı ve post nazal akıntı gibi semptomlara yol açabilirler. Tanı endoskopik muayene, radyolojik ve histopatolojik incelemeyle konulur. Ayırıcı tanıda adenoid kisti, seromusinoz gland kistleri, prevertebral abse, meningosel ve meningoensefalosel akla gelmelidir. Bu çalışmada semptomatik Thornwaldt kisti tanısı almış üç hastaya yaklaşımlarımız literatür eşliğinde sunulmuştur.Anahtar kelimeler: Nazofarengeal kitle, Thornwaldt kisti, manyetik resonans inceleme ABSTRACT Thornwaldt's bursa, also known as nasopharyngeal bursa, is a recess in the midline of the nasopharynx,which is made up by persistent notochord remnants. If the opening of the bursa is occluded, benign midline nasopharyngeal mucosal cyst called Thornwaldt's cyst develops. Although Thornwaldt's cyst is frequently seen, nasopharyngeal obstruction with the mass is a rare condition Cysts are almost always asymptomatic. However, if they become infected or exposed to trauma they can cause some symptoms including halitosis, occipital headache and postnasal drip. Radiological and endoscopic examination can be used to diagnose the cyst. In its differential diagnosis seromucinous gland cysts, intraadenoid cysts, prevertebral abscess, meningocele or meningo-encephalocele should not be forgotten.
