Thoracolumbar fascia mobility and chronic low back pain: Phase 2 of a pilot and feasibility study including multimodal chiropractic care

Abstract: Background Thoracolumbar fascia mobility observed with ultrasound imaging and calculated as shear strain is lower in persons with chronic low back pain. This pilot and feasibility trial assessed thoracolumbar shear strain in persons with chronic low back pain following spinal manipulation and over an 8-week course of multimodal chiropractic care. Methods Adults self-reporting chronic low back pain ≥ 1 year participated between September 2019 and Ap… Show more

“…of the multifidus muscle or of motor control, which are recommended for diagnosis, demonstrating their suitability as a direct examination technique for the clinical appraisal of TLFD 30,39,40 . While most US studies that quantified the gliding or deformation of the TLF did not test the method for reliability 8,22,26,41,42 , Langevin et al 12 controlled the intra-rater reliability of their calculation method and found an ICC of .98, which was slightly higher than our results.…”

“…The TLF shear strain was computed using 2 methods. Found the better effect on this multimodal chiropractic care, as well as the Shear strain increased in females over the same timeframe, but not in males [36].…”

“…In clinical routine, it is much easier to put one's knowledge into practice with patients with ascertained adhesions, suspected presence of peritoneal neoformations due to previous injuries or traumas, or inflammations/infections. In patients with confirmed presence of peritoneal adhesions, manual treatment should not have the aim of breaking the adhesions or creating micro-lesions, as not only is the inflammatory response further stimulated but, above all, the adhesions are not broken with manual medicine [ 3 , 21 ]. Manual medicine should implement very delicate techniques in the area of peritoneal adhesions in order not to stimulate the inflammatory response, in which the immune response is always strongly reactive to mechanical stress [ 3 ].…”
