Thoracodorsal Artery Scapular Tip Autogenous Transplant

Abstract: The Tdast is an excellent choice for reconstruction in the head and neck as an alternative to procedures requiring vein grafting and multiple free tissue transplants, or in which the 3-dimensional contour of the scapular tip aids in reconstruction. The complication rate should be assessed in the context of the risk factors of the patient population and the outcome with respect to stable employment, increasing body mass index, and maintenance of shoulder function.

“…It was first described by Deraemaeker in 1988 and since then it has been increasingly used 11 . In fact, it ensures a number of advantages with respect to other composite flaps 2,9 . For instance, its pedicle can reach up to 20 cm, which is particularly important in maxillary reconstruction where there is a need to avoid vein or artery graft.…”

“…Moreover, each part of the flap receives an independent pedicle with mobile tissue components. This notably facilitates the flap set in 2,9,11 . Other advantages of this reconstructive resource include easy remodeling and fixation.…”

“…Osseous reconstruction is often required to provide structural competence for anterior maxillary projection and along the inferior orbital rim and/or orbital floor. The scapula has often been considered the technique of choice for reconstructing total maxillectomy defects [35,43] . Recent popularization of the thoracodorsal artery scapular tip flap, initially described by Chepeha et al [44] , permits the harvest of the versatile scapular tip with a separate, flexible soft tissue component to recontour the complex craniofacial skeleton.…”
