Thoracodorsal artery as a free graft for coronary artery bypass grafting

Abstract: Arterial myocardial revascularization using different arterial conduits as mammary, radial, gastroepiploic, subscapular and epigastric arteries are well documented. This report describes a preparation and use of thoracodorsal artery as a free graft for coronary artery bypass grafting. The preparation and removal of thoracodorsal artery were performed through right axilla. The artery was used as a free conduit for left anterior descending artery (LAD) in a 58-year-old female for the second revascularization. Th… Show more

“…The thoracodorsal axis has previously been used in coronary artery bypass-grafting. [5] Bifurcated artery-grafts such as the one we describe offer several advantages to vein-grafts. We needed to bypass to both anterior cerebral arteries, and therefore a long Y-shaped graft connected to the carotid would be ideal.…”

“…In two case reports a sternotomy was used for reoperative CAB in one and minimally invasive direct CAB for an initial operation in the other [72]. If a left thoracotomy is not used then the optimal approach to the subscapsular thoracodorsal artery (STA) is via an axillary incision [73]. Mills anastomosed the STA to the descending aorta which may require a pericardial patch; otherwise composite grafting is necessary for this short graft (range, 10 to 12 cm).…”

“…Barner ve ark., (7) 12 yıllık sürede 1000 hasta üzerinde yaptıkları çalışmada ATI ve VSM açık kalma oranlarını sırasıyla 1. yılda %95 -%93, 5. yılda %88 -%74 ve 10. yılda %83 -%41 olarak bildirmişlerdir. Arteria thoracica internadaki bu üstün-lük nedeniyle a. radialis (AR), a. gastroepiploica dextra (AGED), a. epigastrica inferior (AEI), a. splenica, a. subscapularis, a. mesenterica inferior, a. circumflexa femoris lateralis'in r. descendens'i ve a. ulnaris koroner bypass cerrahisinde arteryel greft olarak kullanım alanı bulmuştur (3,(9)(10)(11)(12)(13)(14)(15).
