Thoracic epidural angiolipoma with bilateral multilevel extraspinal extensions: A rare entity

Abstract: Surgical resection is the main treatment for spinal meningioma. The recurrence rate mainly depends on the degree of resection, but complete resection is more difficult to achieve in epidural en-plaque meningiomas because of the location and extent of the lesion, and the attachment to dura mater. Recurrence rates are high, and the prognosis of en-plaque spinal meningiomas remains poor. [2][3][4]

“…Spinal epidural angiolipomas are rare, benign tumors, with only 128 cases reported in the literature till now. [1,2,[6][7][8][9][10][11][12] The most common location of angiolipomas is in the soft tissues of the extremities, trunk, or neck. Berenbruch first described a case of spinal angiolipoma in 1890.…”

“…Therefore, T1-MRI is the standard because it alludes to the presence of a lipid tumor with vascular components resulting in an inhomogeneous picture. [79122125] The level of vascularization correlates with hypointense regions, but iso- or hyper-intense regions may also be appreciated, usually within homogenous intensity in any one patient. The T1-imaging can be used, therefore, to rule out lipomas, which would not have the hypointense regions correlated with vascularization.…”

“…[ 6 18 ] Exacerbation of symptoms may occur during pregnancy and in obese patients probably because of hormonal changes and increase in a fatty component within the angiolipoma. [ 19 ] The CT appearance of spinal angiolipoma is that of a mass that is typically hypodense relative to the spinal cord and has variable degrees of enhancement after contrast administration. [ 20 ] MRI is the investigation of choice in diagnosing spinal angiolipoma.…”
