Thoracic Aortic Endograft Explant: A Single-Center Experience

Abstract: Although rare, familiarity with TEVAR explant is imperative as it is increasingly performed for broader indications. Our limited data suggest that explant can be performed successfully albeit with morbidity.

“…[25][26][27] Reported conversion rates after TEVAR vary between 3.8% and 4.6% with endoleaks, disease progression, retrograde aortic dissection, and aortoesophageal fistulas being the most common cause, which is in line with our series (conversion rate, 4.1%). 12,28,29 There was a lower reintervention rate for emergency cases in this series. This is possibly caused by the higher mortality rate of patients requiring emergency TE-VAR, leaving fewer patients at risk for reinterventions.…”

“…In a cohort of 500 patients with thoracic aortic diseases, 4 patients required stent-graft explantation due to device failure (the deployment system could not be disengaged), aortoesophageal fistula, retrograde AD, and severe type I endoleak (aneurysmal patient). 130 Other reported indications for stent-graft explantation include deployment-related problems, stent-graft infection, migration, collapse, kinking, aortic rupture, and fracture. 39,124,131,132 In the presence of highly unstable hemodynamic conditions, TEVAR has also been proposed as a bridge to open surgery.…”

“…Graft infection is a disastrous complication after aortic repair with reported incidence of 0.2-5% and morbidity rates exceeding 35% [Cappabianca 2017;Heyes 2009;Kirkwood 2010;Smeds 2016;Touma 2014. ] The presence of tracheoesophageal fistula adds to this treatment challenge.…”
