Thirty-day outcome and quality of life after endovascular abdominal aortic aneurysm repair in octogenarians based on the Endurant Stent Graft Natural Selection Global Postmarket Registry (ENGAGE)

Abstract: Octogenarians can be safely treated using the Endurant stent graft with a high technical rate of success, low perioperative mortality, and no reduction in quality of life. Octogenarians did, however, appear to recover more slowly than younger patients with respect to certain quality of life components. Long-term data are needed to confirm these results.

“…The elderly population had a higher peri-operative rate of pulmonary complications (5.1% vs. 1%, P = 0.003) and access-site haematomas (12% vs. 2.4%, P = 0.001) with no difference in stroke, myocardial infarction, death, or ischaemic complications. Pol et al [17] analysed 274 patients from the Endurant Stent Graft Natural Selection Global Postmarket Registry (ENGAGE) vs. 926 younger patients using the EuroQoL-5 Dimensions Quality of Life (QoL) questionnaire. All-cause mortality and major adverse event rate peri-operatively was similar among groups ( P = 0.835 and P = 0.186, respectively).…”

“…This study does not report about the quality of life after operative or conservative treatment of an AAA. Earlier, the EVAR 1 and 2 and the DREAM trial reported conflicting results on outcome of quality of life after EVAR and open repair [5,20,22,24]. They all reported decrease in quality of life after EVAR and open repair of AAAs.…”

“…Indeed, the increase in 30‐day case fatality of acute AAA events with age was much greater in absolute terms than the increase in case fatality owing to elective AAA repair. One quarter of elective AAA repairs in the UK are already performed in patients over the age of 80, with excellent functional outcomes in the vast majority . Second, our estimates of effectiveness of screening took into account the shorter life expectancy in older individuals with known aneurysmal disease.…”
