Theta–gamma coupling as a cortical biomarker of brain–computer interface-mediated motor recovery in chronic stroke

Abstract: Chronic stroke patients with upper limb motor disabilities are now beginning to see treatment options that were not previously available. To date, the two options recently approved by the United States Food and Drug Administration include vagus nerve stimulation and brain-computer interface therapy. While the mechanisms for vagus nerve stimulation have been well defined, the mechanisms underlying brain-computer interface-driven motor rehabilitation are largely unknown. Given that cross-frequency coupling has b… Show more

“…Theta-gamma PAC is highly dependent on proper E/I balance (11,12), with gamma frequency synchrony attributed to interconnected fast-spiking GABA A -expressing interneurons (11) and theta frequency attributed to cortical excitatory neurons and more particularly to layer V pyramidal neurons of the human brain (36, 37). In addition, rodent and human motor cortices were reported to exhibit strong theta-gamma coupling (38)(39)(40). To assess PAC, we chose the method of the modulation index (MI) developed by Tort et al (41,42) for its noise tolerance and amplitude independence.…”

“…previous therapeutic devices have used electrical stimulation to encourage neuroplasticity, she says, but this system couples movement intention to actual movement, providing a much more powerful stimulus for learning and plasticity. In a clinical trial, two-thirds of users achieved clinically significant improvements after six months 2,3 . For instance, says souders, "They're able to grasp something and carry it somewhere."…”
