Thermal ablation techniques: a curative treatment of bone metastases in selected patients?

Abstract: • Thermal ablation techniques are currently performed to palliate pain caused by bone metastases. • In selected patients, thermal ablation can also be indicated with a curative intent. • Oligometastatic and/or metachronous diseases are good prognostic factors for local success. • Small-size (<2 cm) bone metastases and no cortical erosion are good prognostic factors.

“…Minimal morbidity, short recovery time, and repeatability in the same organ are major advantages of these techniques. [72][73][74] Transarterial chemoembolization (TACE) consists of the injection of an emulsion of lipiodol (Guerbet, Aulnay sous Bois, France) and chemotherapy directly into the liver arterial supply. In one retrospective study using platinum-based TACE as second-line treatment, 75 an 83% rate of liver tumor control was reported at 3 months, and this was observed especially in patients with small metastases (3 cm).…”

“…6 Thirty-eight lesions required additional cementoplasty. Definition of "curative treatment" was composite as they divided their population in two groups: in the first one, the goal was to achieve complete treatment of all bone metastases; in the second one, the goal was to prevent skeletal related events (SREs).…”

“…Lesions smaller than 2 cm without cortical erosion had a particularly favorable prognosis. Local tumor-free survival after one year was achieved in 85 % of cases [64]. Supplementary measures such as osteoplasty can be necessary regardless of ablative therapy.…”
