Acute infection with the hepatitis B virus resolves in 95% of adults with no need for specific medical therapy. However, a few cases present as severe hepatitis or acute liver failure, which may require orthotopic liver transplantation or cause death. Under these circumstances, medical therapy that can improve the clinical outcome and improve spontaneous survival would be highly useful; yet, published data regarding the utility of antiviral therapy in acute hepatitis B is quite limited. In this paper, the efficacy and safety of oral nucleoside and nucleotide analogues in the treatment of acute hepatitis B are reviewed, and guidelines for treatment of these acute cases are revisited.

Acute glomerulonephritis (GNA) is a wide group of diseases with the common characteristics of abrupt onset and proliferation of endocapillary glomerular cells. Clinically, they usually manifest as acute nephritic syndrome: hematuria, acute renal failure, hypertension and mild proteinuria, although they can appear as recurrent hematuria. GNA can be present in association with a variety of bacterial and viral infections, but in most cases the initial antigenic initiating stimulus is unknown. Patients with rapidly progressive glomerulonephritis often present acute onset of manifestations of nephritis, such as azotemia, oliguria, edema, hypertension, proteinuria, and hematuria with «active» urine sediment that often contains red blood cell casts, pigmented casts, and cellular debris. This article, aimed at all primary care staff and seeking to increase their awareness of this disease, discusses in more detail the clinical, pathogenesis, pathology, and treatments available from one of the glomerulonephritis with greater impact on survival patient.

Cryoglobulinemic glomerulonephritis in chronic hepatitis B infection

Hepatitis B virus (HBV) infection is an uncommon cause of cryoglobulinemia. Renal cryoglobulinemia has been rarely reported in the setting of chronic hepatitis B infection. We describe a case of chronic hepatitis B infection with cryoglobulinemic glomerulonephritis (Gn) and provide information about the treatment and the evolution over a 30-month follow-up. A 41-year-old woman with chronic hepatitis B infection developed nephrotic syndrome and acute renal failure; other investigations revealed type 2 cryoglobulinemia; HBV DNA was detected in the cryoprecipitate. Renal biopsy showed findings of cryoglobulinemic Gn. She was given lamivudine, corticosteroids, plasma exchange, and mycophenolate mofetil. The renal function improved, nephrotic syndrome remitted, and HBV DNA became undetectable; there was no compromise of the liver function.
