Therapy of the Postinfectious Irritable Bowel Syndrome: An Update

Abstract: After acute infectious gastroenteritis, up to thirty percent of patients present prolonged gastrointestinal symptoms and a part of those affected patients can have the diagnostic criteria for postinfectious irritable bowel syndrome.Treatment is symptom directed rather than curative and includes agents prescribed for the treatment of irritable bowel syndrome in general. Prophylaxis or early treatment of acute bacterial diarrhea may reduce the risk of postinfectious irritable bowel syndrome development by reduci… Show more

“…The mechanisms by which gut microbes influence the CRC tumorigenesis (Iacob et al, 2017) were actively under study. For examples, researchers have recently learned that the gut microbiota plays a regulatory role in the tumor microenvironment and thus in tissue carcinogenesis (Sohn et al, 2015; Nagy-Szakal et al, 2017; Morgillo et al, 2018).…”

“…The characteristics of the infectious illness such as diarrhea, abdominal cramps, increased stool frequency, bloody or mucous stools, and positive stool culture and weight loss are potent predictors of long term outcome. The risk of PI-IBS appears to correlate with the severity of the acute enteric infection [12,13]. The above mentioned symptoms are frequently associated with CDI.…”

“…The postinfectious irritable bowel syndrome (PI-IBS) is characterized by the sudden onset of symptoms mentioned in the diagnostic criteria for irritable bowel syndrome that appear after an episode of acute infectious gastroenteritis with by two or more of the following symptoms and findings: diarrhea, vomiting, fever and a positive stool culture result [27]. Pathophysiologic mechanism for PI-BS may be associated with altered motility, increased intestinal permeability, increased numbers of enterochromaffin cells and persistent intestinal inflammation, characterized by increased numbers of T-lymphocytes and mast cells, and increased expression of proinflammatory cytokines.…”
