Therapy of large multinodular goitre using repeated doses of radioiodine

Abstract: In most cases of LMG, repeated administration of radioiodine is safe and effective. The highest response of the thyroid volume is observed after the first course of treatment. On account of a high incidence of hypothyroidism, the patients should be monitored during and after therapy.

“…As per the standard WHO correction, the mean volume of 912 lobes was 33.25 ± 17.32 (5.30 Based on the cut off volume (50 cc) for each lobe, they can be categorized as small goitres (B50 cc) and large goitres ([50 cc) [7]. The mean volume of small goitres was 26.45 ± 11.43 (5.30-49.80) cc and that of large goitre was 60.89 ± 5.96 (50.30-76.90) cc.…”

“…Ex vivo specimen will have reduced volume compared to pre-operative or intra-operative levels due to shrinkage in size after vascular cut-off. The volume of each thyroid lobe was calculated separately, Individual lobe with volume more than 50 cc in ex vivo specimen was labelled as large goitre where as lobe with volume of 50 cc or less are labelled as small goitres [7]. Based on the presence of hyperthyroidism, the cases are divided into toxic and non-toxic cases.…”

“…It is undoubtedly the method of choice for young patients without any concomitant diseases. In patients with a toxic goitre antithyroid drug treatment resolves metabolic problems, but still does not influence thyroid reduction; on the contrary it stimulates further goitre growth [1]. Apart from those patients who give their consent to surgery and do not have any contraindications, there are also those who need treatment but definitely should not be operated on.…”

“…In toxic goitre one of the options is antithyroid drug treatment. Nevertheless, it cannot be used permanently due to its negative impact on the thyroid volume, causing further goitre growth [1,2]. 131-I treatment, especially in nontoxic goitre, has thus far been reserved mainly for elderly patients, whose numerous concomitant diseases disqualify them from surgery [3,4].…”
