Therapy for Foetal Pericardial Tumours: Survival following in utero Shunting, and Literature Review

Abstract: We report the first case of foetal pericardial teratoma, treated successfully in utero by pericardio-amniotic shunting, and review the relevant literature. Foetal treatment improves survival in case of hydrops. Foetal pericardio-amniotic shunting could possibly be an alternative to serial pericardiocentesis.

“…Twenty-one (38.2 %) fetuses had critical or severe conditions in utero or after delivery: fetal/neonatal respiratory distress (n = 12) [12-14, 19, 21, 22, 29, 33, 34, 38, 50], cardiac tamponade (n = 3) [16,17,23], fetal arrest (n = 3) [11,37,46], dyspnea (n = 2) [27,39], and tachypnea (n = 1) [38].…”

“…However, there is only scarce data regarding this procedure in the literature and it is highly [8,17]. Table I [8,17], two cases underwent pericardioamniotic shunting [9,10] and one case underwent thoracoamniotic shunting following pericardiocentesis [7]. In others, single or serial pericardiocentheses were performed for hydrops or tamponade or large pericardial effusion [3,4,6,8,12,15,16,[18][19][20][21][22].…”

“…Thereby, we identified 5 cases with pericardial teratoma in which a pericardio-amniotic shunt was successfully placed [5,6,7,8,9]. One fetus died in utero 8 days after the intervention and one died due to premature delivery 1 1 / 2 weeks after the intervention.…”
