Therapeutic relationships: from psychiatric hospital to community

Abstract: The objective of this study was to determine the cost and effectiveness of a transitional discharge model (TDM) of care with clients who have a chronic mental illness. The model was tested in a randomized clinical trial using a cluster design. This model consisted of: (1) Peer support for 1 year and (2) Ongoing support from hospital staff until a therapeutic relationship was established with the community care provider. “…The purpose of this study was to fill gaps in the literature related to peer support programs and cost: lack of comparison groups, small sample sizes (Forchuk, Martin, Chan, & Jensen, 2005), and the availability of research examining mental health utilization paid for by Medicaid (Mark et al, 2011). This study extends the author's previous analysis of the Georgia peer support program by examining the relationships between the utilization of peer support services and cost.…”

“…Typically, such transitions are fraught with the challenge of poor communication between in-and outpatient providers working in siloed systems, along with high rates of relapse, particularly in the first month postdischarge. 59 This has led to the development of transition-focused interventions, several of which embed recovery principles in the use of peer support and the emphasis on community engagement, 60,61 or in the use of rehabilitation-focused partial hospitals that facilitate earlier discharge and reduce admissions. 62 Such interventions show considerable promise in reducing rates of rehospitalization and improving community participation.…”

“…For a general population of psychiatric patients that included forensic patients, a transitional discharge model which involved peer support and the establishment of a relationship with a community nurse was successful in reducing the length of stay of patients and improving quality of social relations (Forchuk, Martin, Chan, & Jensen, 2005). The importance of community support in reducing readmission of psychiatric patients to the hospital, diminishing symptoms, and increasing ability to live in the community has also been supported by other research (Reynolds et al, 2004).…”
