Therapeutic plasma exchange in patients with COVID-19 pneumonia in intensive care unit: Cases series

Abstract: Introduction: COVID 19 pneumonia can lead to an inappropriate inflammatory response, and can be complicated by acute respiratory distress syndrome, multivisceral failure with a high mortality rate. Objective: To observe the effect of therapeutic plasma exchange on the excessive inflammatory response. Materials and methods: In this study, we included 7 confirmed cases of COVID-19 in the intensive care unit (ICU) department of the university hospital of Oujda. COVID-19 cases were confirmed by RT PCR (reverse tra… Show more

“…TPE was usually performed multiple times, averaging three times during hospitalization and ranging from one to seven times. The main component of replacement fluid was fresh frozen plasma in all 13 studies, or FFP with a combination of citrate dextrose solution [ 19 ], human albumin and normal saline [ 21 ], normal saline only [ 22 , 29 ], or 5% human albumin only [ 23 , 24 , 25 , 26 , 27 , 30 ]. Regarding the safety of TPE, only one study reported one patient with associated hypotension [ 19 ], whereas another study reported femoral artery puncture in one patient and thrombophlebitis of the femoral vein in another patient [ 22 ].…”

“…Previous studies have demonstrated that TPE can improve the outcomes of patients with COVID-19 with a high risk of thrombosis or ARDS [ 24 28 ]. In this case report, the patient’s respiratory and neurological symptoms improved after five cycles of TPE.…”

“…In patients with severe COVID-19, TPE might be considered an efficient therapy for reducing the risk of cytokine storm-induced ARDS as it might reduce the inflammatory cytokines (including IL-6) and acute phase proteins (ferritin and CRP). This could help in improving tissue oxygenation and reducing the risk of hypercoagulation and improve tissue oxygenation and especially fatal cardiovascular events ( 37 , 38 ).…”
