Therapeutic Plasma Exchange as a Bridge to Total Thyroidectomy in Patients with Severe Thyrotoxicosis

Abstract: Objective: Graves disease is the most common cause of thyrotoxicosis. Medical management is the first-line treatment but may be contraindicated or ineffective. In patients with severe, refractory thyrotoxicosis therapeutic plasma exchange (TPE) may be indicated as a bridge to thyroidectomy. Methods: We present 3 cases of thyrotoxicosis refractory to medical management that were successfully treated with TPE and subsequent total thyroidectomy, and provide an analysis of the response to therapy via a change in f… Show more

“…24 TPE treatment has been described in 91 non-pregnant individuals with GD. [25][26][27][28][29][30] Use of TPE for hyperthyroidism in pregnant women [31][32][33] or during molar pregnancies [34][35][36] has only been rarely reported. No controlled trial has been performed.…”

“…Other alternatives have been proposed in such cases, such as cholestyramine and plasma exchange for the rapid development of euthyroidism before surgery [6] [12] [13]. Furthermore, a herbal decoction of Anemarrhena Bounge is potentially effective in patients with resistance to ATDs and is a therapeutic avenue warranting future exploration [5].…”

“…2 In patients with severe hyperthyroidism, in cases of thyroid storm and in cases where ATD cannot be used due to side effects or ineffectiveness, plasmapheresis therapy can be used to provide euthyroidism before permanent treatment. [3][4][5][6][7][8] Therapeutic plasma exchange (TPE) is the process of replacing patient plasma with albumin or fresh frozen plasma (FFP). During this treatment, thyroid hormones, high molecular weight substances, autoantibodies, immune complexes, cytokines, catecholamines and thyroid-binding globulins are also cleared from the patient's blood.…”
