Therapeutic Mammaplasty for Breast Cancer: Oncological and Aesthetic Outcomes

Abstract: Primary chemotherapy allowed for TM in patients with large tumours. Intraoperative margin assessment decreased reoperation rate. Contralateral matching procedures resulted in histological detection of occult disease. TM is an oncologically appropriate and cosmetically favourable technique.

“…This recurrence rate is comparable to other published studies of oncoplastic mammoplasty patients. 5,6,11,[34][35][36] When we anticipate that there will be a significant tumor burden left at the end of neoadjuvant therapy, and thus the likelihood of radiation is high, even if a mastectomy is planned, we try to perform breast conservation if at all possible to minimize reconstructive complications. “…Oncoplastic breast‐conserving surgery may offer oncological advantages as a result of the wider area of tissue excised and, importantly, margins are more likely to be clear with a reduction in the number of second operations required. In a survey of 11 studies with a total of 1532 patients undergoing therapeutic mammoplasty, a median of 7·4 (range 0·1–16) per cent underwent further surgery for close or involved margins.
