Background: Hypothermia contributes to morbidity and mortality of newborns. While there have been improvements in neonatal mortality both globally and nationally, there are still important regional differences. Adopting prevention and intervention practices to reduce hypothermia at birth may help achieve the global and national goal of reducing neonatal mortality. Purpose: To assess the contribution of admission hypothermia to mortality among hospitalized newborn infants. Methods: Retrospective cohort study was conducted in Sheik Hassan Yabare Jigjiga University Referral Hospital. Neonates were admitted to the Neonatal Intensive Care Unit were selected by using simple-random sampling technique from record of neonates. Descriptive survival analysis such as Log rank test, life table and Kaplan-Meier survival curve and bivariate and multivariate inferential Cox regression were used to estimate hazard ratios with 95% confidence intervals. Results: Among 588 neonates, 146 (24.8%) died and 442 (75.2%) were censored. A total of 2509 days were followed for hypothermic neonates and 2337 days for normothermic neonates. The death rate was 37 per 1000 days and 22 per 1000 days for hypothermic and normothermic neonates, respectively. Survival function between the two groups differed significantly.

Birth asphyxia is a separate nosological form characterized by clinical symptoms ofcardiorespiratory and neurological depression of a newbornwith possible subsequent development of encephalopathyand multiorgan dysfunction, as well as laboratory signsof side effects of fetal hypoxia before or during childbirth(significant metabolic or mixed acidosis in umbilical cordblood gases). The current method of treatment of childrenwith manifestations of asphyxia is therapeutic hypothermia,that should be accompanied by continuous monitoring ofthe patient's cerebral functions.The article presents a clinical case of severe asphyxiain the full-term boy born by cesarean section due to fetaldistress and effusion of meconium amniotic fluid. The scoreon the Apgar scale was 1 point at the end of the first minuteof life, and 3 points five minutes after birth. Therapeutichypothermia by passive cooling was started at the maternityhospital and continued in the intensive care unit of NationalChildren’s Specialized Hospital “OKHMATDYT” of theMinistry of Health of Ukraine (Kyiv, Ukraine).The child was getting dynamic complex neuromonitoringthat included clinical assessment of neurologicalstatus, cerebral oximetry by near-infrared spectroscopy,neurosonography, magnetic resonance imaging, videoelectroencephalographic monitoring and polysomnography.The results of a comprehensive examination of the childmade it possible to identify functional markers of cerebraldisorders: cortical rhythm depression, electro-clinicalseizures, changes in oxygenation of brain tissues, signs ofleukomalacia and ischemic damage, etc. Polysomnographicsigns of sleep-related breathing disorders were found outwith the development of mild apnea-hypopnea syndrome,sinus bradycardia, delayed maturation of physiologicalsleep precursors. Delayed motor development withoutstructural epilepsy were noted at follow-up at the age of2 months.

Background: The brain damage caused by perinatal hypoxic-ischemic encephalopathy (HIE) can have fatal implications for the baby. Wingless-related integration site (Wnt) signalling is crucial for embryonic cell proliferation, fate determination, and patterning, as Dickkopf-1 (DKK-1) is a secreted protein that plays a role in these processes. Objective: To understand the role of Dkk-1 in the prognosis of newborns with HIE. Patients and Methods: From November 2019 to April 2020, at the neonatal intensive care unit (NICU), Menoufia University Hospitals we undertook a case-control research. Two groups of patients: Asphyxiated 15 newborns were included in patient group (Group 1). Group 2: 15 newborns in good health. Blood count, liver and kidney function test, and blood gas analysis were performed. CT or MRI scans were performed. Serum DDK-1 levels at admission and discharge were measured. Results: In newborns with HIE, the DKK1 level was greater than in normal neonates. DKK-1 level correlated positively with degree of HIE. DKK-1 level correlated positively with worse outcome among HIE neonates. Serum -level >27 µg /L with a sensitivity of 100% and a specificity of 100% for prediction of mortality in the studied HIE neonates. Conclusion: There were association between serum DKK-1 level as diagnostic factor and if an Apgar score was less than 3 at the 5 th minute, pH less than 7.0, or base excess (BE) less than 12 in the venous or cord blood of neonates within 60 minutes of delivery, asphyxia was diagnosed as a prognostic factor in neonates having HIE.
