Therapeutic Drug Monitoring of Olanzapine: The Combined Effect of Age, Gender, Smoking, and Comedication

Abstract: Therapeutic drug monitoring (TDM) data for the antipsychotic drug olanzapine were investigated with respect to concentration versus dose relationship, intraindividual versus interindividual variability, and the combined influence of patient characteristics on steady-state concentration. The study included 250 patients, with daily doses ranging from 2.5 to 30 mg. Median concentration to dose ratio was 2.1 (ng/mL)/(mg/d), with 90% of the distribution in a fivefold range. In the first subgroup of patients with tw… Show more

“…Previous reports are contradictory on these factors. 16,24,25,28,29 This study was not designed as an interaction study and the majority of patients were on polypharmacy. Thus, smaller effects may have been missed.…”

“…Therefore, the concentration of these drugs in plasma is very low (pg to ng/mL levels). For example, the therapeutic plasma levels of OLZ are in the range of 8-80 ng/mL (Olesen and Linnet, 1999;Gex-Fabry et al, 2003). In the case of HAL levels as low as 2.0 ng/mL have been described as within the therapeutic range, though most references indicate that 5.0 ng/mL represents the lower end of the therapeutic range.…”
