Therapeutic Biliary Endoscopy

Abstract: Some steps of progress have been seen in the area of biliary therapeutic endoscopy, in the understanding and management of primary sclerosing cholangitis, problems relating to liver transplantation, malignant biliary strictures, and complications after endoscopic retrograde cholangiopancreatography, as well as sphincter of Oddi dysfunction. These topics are reviewed here.

“…E NDOSCOPIC BILIARY STENTING under endoscopic retrograde cholangiopancreatography (ERCP) is the first-line therapeutic modality to decompress biliary obstruction; however, it has been noticed to fail in up to 10% of patients, usually as a result of anatomical variations, duodenal strictures, and previous gastrointestinal surgeries. 1,2 Since its first report in 2001, 3 endoscopic ultrasound-guided biliary drainage (EUS-BD) has emerged as a new biliary decompression procedure and has been increasingly used as an effective alternative for biliary obstruction patients with failed ERCP. 4,5 In the last decade, EUS-BD has been attracting considerable attention and various endoscopic techniques have been described for its implementation.…”

“…S TENT PLACEMENT UNDER endoscopic retrograde cholangiopancreatography (ERCP) is the gold standard method of treating malignant biliary obstruction. 1 This method may be contraindicated for duodenal obstruction or surgical anatomy such as Roux-en-Y reconstruction because the endoscope cannot be advanced into Vater's ampulla. As an alternative method, percutaneous transhepatic biliary drainage (PTBD) is widely carried out.…”

“…Endoscopic retrograde cholangiopancreatography (ERCP) is the standard procedure, with a high success rate, for the treatment of biliary obstruction [Fogel et al 2001; Freeman and Guda, 2005]. However, ERCP may not be possible in patients with selective cannulation failure or an inaccessible papilla due to a surgically altered anatomy or duodenal obstruction [Park et al 2011; Ogura et al 2014].…”
