The Value of PSA, Free-to-total PSA Ratio and PSA Density in the Prediction of Pathologic Stage for Clinically Localized Prostate Cancer

Abstract: While PSAD is more helpful than PSA and free/total PSA ratio for prediction of organ-confined disease, none of these parameters are significant predictor of pathologic stage for clinically localized prostate cancer.

“…Although the Prostate-Specific Antigen (PSA) is a very good biomarker for prostate cancer, attempts to discriminate prostate cancer patients from a control group using the dataset reported by Petricoin et al [280] show that PSA correctly classifies only 62% of cancer patients. Similar results have been reported by other authors [281]. An explanation for this fact is that diseases are multi-causal phenomena so we should expect that the use of more than one biomarker may improve diagnostics.…”

“…Lam et al showed that PSAD at cut-off value of 0.15 ng/mL/mL spread 50% of men from undergoing unnecessary biopsies ( 11 ). In addition, some investigators have shown that PSAD is more specific compared to free to total PSA ratio especially when serum PSA level is at lower range ( 12 - 14 ). In our study, TZPSAD was found to be even more specific compared to PSAD and spread more patients from undergoing unnecessary biopsies.…”
