The value of prostate specific antigen (PSA) density and free: total PSA ratio in selecting patients with a normal digital rectal examination and intermediate total PSA levels for further investigation

Abstract: Objectives  To examine the use of prostate‐specific antigen (PSA) density (PSAD) and free to total PSA ratio (f/tPSA) in enhancing the specificity of PSA in the diagnosis of prostate cancer in patients with a total PSA (tPSA) of 4–10 ng/mL and with a normal digital rectal examination (DRE). Patients and methods  The study comprised 77 consecutive men in whom the fPSA and tPSA levels were obtained before DRE and transrectal ultrasonography‐guided sextant prostate biopsy. Prostate cancer was found in 39 patients… Show more

“…Our results were slightly higher, therefore, for the detection of PCa using PSA [15]. For patients with total PSA 4-10 ng/ml, there are authors [17,18] who, using PSAD in order to diagnose PCa, obtained a significant increase in specificity with a minimum reduction in sensitivity. In our series, by just applying a PSAD threshold of 0.20, we managed to increase the specificity for total PSA, but with an unacceptable reduction in sensitivity (44%) and leaving undiagnosed 60.5% of Pca.…”

“…determined that ratio of free to total PSA in patients with carcinoma prostate is significantly lower than in men with BPH. Klinger et al 1 . showed that free: total PSA ratio was better diagnostic marker than total PSA.…”

“…However, values 4-10 are suspicious and other subsidiary parameters such as percentage of free PSA (the ratio of free PSA to total PSA), PSA velocity, and PSA density, contribute to the diagnosis and decision making about prostate biopsy. [ 1 5 6 7 8 ]…”
