The utility of intravascular ultrasound compared to angiography in the diagnosis of blunt traumatic aortic injury

Abstract: CTA is useful as a screening test in suspected TAI. When additional imaging is required after an equivocal CTA, IVUS is better than angiography. Therefore, we advocate the use of IVUS in potential TAI patients in whom angiography is being considered.

“…Since intravascular ultrasound has been described as a useful and accurate diagnostic tool in case of TEVAR for BTAI, 8 it may have helped us to detect the distal graft infolding during follow-up, perhaps allowing early endovascular reintervention.…”

“…IVUS, like helical CT but unlike aortography, can visualize the lumen and both the aortic wall and the periaortic structures. Although limited by the absence of a reference-standard technique, a recent study found that IVUS performed better than catheter 323 A decided disadvantage of IVUS is its limited field of view. In addition, the high cost of disposable transducers and the invasive nature limit IVUS to a problem-solving tool at present.…”

“…IVUS is not appropriate as a screening test, but it can provide detailed morphologic information that can guide the choice of therapeutic devices (stent grafts) for the endovascular treatment of TAI. 17 IVUS can be used to improve the pseudoaneurysms of extremity arteries may be managed with US-guided thrombin injection, a technique that is commonly used to treat iatrogenic femoral artery pseudoaneurysms, but it can also be used for other arterial pseudoaneurysms (Fig. 6-5).…”
