Background: Drilling in neurosurgery is an integral part of surgical exposure especially in skull base approaches and Cranio-Vertebral Junction (CVJ) surgeries. Most of such drillings are done in the close proximity of the neurovascular structures in skull base surgeries and Cervical-Medullary Junction (CMJ) or facet/pedicle in CVJ surgeries. Reluctance to drilling in young neurosurgeons are due to less hands on experience during training and also, in the early part of the career, due to fear of injury to neurovascular structures. Methods: Five commonest bone removals for skull base region and CVJ surgeries were identified which can be safely done using manual instruments based of experiences of senior authors. Authors highlight key technical nuances to widen surgical corridors using manual instruments safely for skull base surgical approaches. Results: Basic neuroanatomical concepts and basic physics help in using manual instruments safely for bone removals in various skull base surgical approaches. Conclusions: Manual instruments may be used for bone removals in selected skull base surgical approaches which help young neurosurgeons to perform these surgeries in limited resource settings.

BACKGROUND Neurosurgery residency programs are tasked with imparting large volumes of both clinical knowledge and technical skill to trainees in limited time. Many investigators have described local practices, which may offer evidence-based interventions in neurosurgical residency education, but this literature has not been systematically reviewed. OBJECTIVE To perform a scoping review of educational practices in neurosurgery, which are supported by quantitative, peer-reviewed research. METHODS A scoping review of the literature was performed. PubMed, Embase, and Web of Science databases were queried for articles describing educational interventions for neurosurgery residents, which included a quantitative assessment of the effect on resident performance. RESULTS From an initial set of 1785 unique articles, 29 studies were ultimately screened and included. Studies were into the following 6 topics: (1) didactics and curricula (n = 13), (2) nontechnical skills (n = 6), (3) wellness and burnout (n = 4), (4) assessment and feedback (n = 2), (5) mentorship and career development (n = 2), and (6) research (n = 2). Individual study results were described. CONCLUSION Several educational interventions in neurosurgical training are supported by quantitative evidence. Methodological shortcomings are prevalent among studies of education, particularly in the selection of meaningful outcome measures. A summary of evidence-based considerations is provided for current and future program directors.

Since its inception more than 12,000 years ago, neurosurgery has progressed through a number of phases, largely dictated by the available supporting bodies of information and technical adjuvants that allowed progress. The recent rapid escalation of technical progress related to surgery of the nervous system as well as global and regional climates have created diverse demands for the field and have challenged traditional methods of education for developing needed neurosurgical expertise. The creation of the "new neurosurgeon" is, in fact, the creation of many individuals and is dependent on objectives that are based on the needs directed toward the field. These needs are local, national, and international, and they are created by medical, scientific, economic, social, and political factors. Specific education should be directed toward these needs from early educational involvements at the premedical level but also should pervade the entire medical specialty exposure and postgraduate training. This communication discusses specific categories of needs and makes suggestions regarding programs and involvements that may address elements of the situation. General information and educational principles are discussed, along with recommendations for the fostering of a climate of innovation for the field.
