The usefulness of computed tomography in the diagnosis of impacted fish bones in the oesophagus

Abstract: The usefulness of computed tomography (CT) in the diagnosis of fish bone impaction in the oesophagus was evaluated. Thirty-two patients were examined by plain X-ray followed by direct oesophagoscopy for suspected fish bone impaction. Among 25 cases in which fish bones were actually removed, foreign bodies were not clearly demonstrated by plain X-ray in 14 cases (56 per cent). Eleven cases underwent CT prior to the oesophagoscopic examination. Fish bones were clearly demonstrated by CT in all patients. CT also … Show more

“…Radiological opacity is not uniform even within one species. (11,12,13) This is also observed in the study of Evans et al, Watanae et al, Eli and Palme. (14) According to Palme, they detected 25 to 55% cases of fish bones in X-ray.…”

“…Certain reports have demonstrated the usefulness of CT for detecting impacted fi sh bones in the esophagus with a sensitivity of 90.9%-100% and specifi city of 100% using CT images taken at thin intervals. 1,5,8 Moreover, CT not only confi rms the existence and location of any impacted fi sh bones, it enables visualization of any resulting damage to neighboring structures. 5 Generally, patients with sharp objects lodged in the esophagus require urgent endoscopic intervention.…”

“…1 Esophageal foreign body impaction usually presents acutely, especially in adults, who normally have a clear history of ingestion. Adult patients may have diffuse chest pain, pressure, a vague foreign body sensation, a sensation of choking, or neck or throat pain.…”

“…Thus, physicians should not rely on this modality alone to either confirm or rule out any suspected fish bone impaction (8). Persistence of symptoms is an indication for CT scan as it may become a reliable adjunct tool, whereby in a retrospective Japanese series reported the sensitivity for a CT to pick up the FB was 100%, as compared to the 44% for plain films (9). In certain cases, they can get embedded into the esophageal wall and cause persistent odynophagia.…”
