Рак яичников занимает лидирующее положение по показателю смертности среди всех злокачественных новообразований репродуктивной женской системы. Тактика лечения впервые выявленного рака яичников заключается в выполнении максимально возможной циторедуктивной операции в сочетании с комбинированной химиотерапией. Несмотря на высокую эффективность 1-й линии химиотерапии, примерно у 10% пациентов прогрессирование заболевания развивается в ходе лечения, а у 55–75% – в первые 2 года после окончания лечения. Выбор режима лекарственной терапии рецидива рака яичников в первую очередь зависит от времени наступления прогрессирования опухолевого процесса. Eсли прогрессирование заболевания возникло более чем через 6 месяцев с момента последнего введения препаратов платины (платиночувствительный рак яичников), то таким пациентам показано назначение карбоплатина/цисплатина в сочетании с другим противоопухолевым препаратом, не назначавшимся ранее, а если после завершения лечения прошло более 2 лет – возвращаются к тому же режиму, который применялся изначально. В случае наступления прогрессирования опухолевого процесса менее чем через 6 месяцев с момента последнего введения препаратов платины (платинорезистентный рак яичников) назначаются неплатиновые лекарственные средства в виде как монотерапии (паклитаксел, топотекан, гемцитабин, пегилированный липосомальный доксорубицин и др.), так и дуплетов. Добавление к лечению бевацизумаба и PARP-ингибиторов в ряде случаев приводит к увеличению времени до прогрессирования и общей выживаемости. Ovarian cancer takes a leading position in terms of mortality among all malignant neoplasms of the female reproductive system. The tactic of treating newly diagnosed ovarian cancer is to perform the maximum possible cytoreductive surgery in combination with chemotherapy. Despite the high effectiveness of the first line of chemotherapy, approximately 10% of patients develop disease progression during treatment, and 55– 75% of patients – in the first two years after the end of treatment. The choice of a drug therapy regimen for ovarian cancer recurrence primarily depends on the time of onset of the progression of the tumor process. If the progression of the disease occurred more than 6 months after the last administration of platinum preparations (platinum-sensitive ovarian cancer), then such patients are shown the appointment of carboplatin/cisplatin in combination with another antitumor drug that was not prescribed earlier, and if more than two years have passed after the completion of treatment, they return to the same regimen that was used initially. In case of progression of the tumor process less than 6 months after the last administration of platinum preparations (platinum-resistant ovarian cancer), non-platinum drugs are prescribed as monotherapy (paclitaxel, topotecan, gemcitabine, pegylated liposomal doxorubicin, etc.) and in the form of duplets. The addition of bevacizumab and PARP inhibitors to treatment in some cases leads to an increase in the time to progression and overall survival.

Various in-vitro chemosensitivity and resistance assays (CSRAs) have been demonstrated to be helpful decision aids for non-neurological tumors. Here, we evaluated the performance characteristics of two CSRAs for glioblastoma (GB) cells. The chemoresponse of fresh GB cells from 30 patients was studied in vitro using the ATP tumor chemoresponse assay and the chemotherapy resistance assay (CTR-Test). Both assay platforms provided comparable results. Of seven different chemotherapeutic drugs and drug combinations tested in vitro, treosulfan plus cytarabine (TARA) was the most effective, followed by nimustine (ACNU) plus teniposide (VM26) and temozolomide (TMZ). Whereas ACNU/VM26 and TMZ have proven their clinical value for malignant gliomas in large randomized studies, TARA has not been successful in newly diagnosed gliomas. This seeming discrepancy between in vitro and clinical result might be explained by the pharmacological behavior of treosulfan. Our results show reasonable agreement between two cell-based CSRAs. They appear to confirm the clinical effectiveness of drugs used in GB treatment as long as pharmacological preconditions such as overcoming the blood-brain barrier are properly considered.
