The Use of LUMIC Prosthesis for the Treatment of Periacetabular Metastases

Abstract: 1. Surgical intervention in patients with periacetabular metastases often requires the use of modular or custom-made implants. 2. Radiotherapy is a complementary treatment or is reserved for use in patients in poor general health. 3. An alternative to the Lumic prosthesis is a custom-made prosthesis produced with 3D printing technology.

“…The main aim of periacetabular reconstruction is to achieve a functional hip joint and a stable lower extremity. Guzik 19 reported mean postoperative physical function 65 in the Karnofsky scale in a series of six patients who underwent resection of periacetabular area metastases followed by implantation of a LUMiC® prosthesis. The functional status of the patients with custom‐made implants has varied from 50% to 66.6% in evaluation by MSTS scores in different studies 20‐22 .…”

“…This can be achieved using an anti-protrusion acetabular cage with screws placed in an antegrade or retrograde mode[ 42 , 43 ]. When there is extensive bone loss prohibiting the aforementioned technique, the skeletal defect may be bridged using a pelvic prosthesis (usually an ice-coned device) that is docked to the remaining intact pelvic bone, a technique which usually requires adequate bone stock around the posterior iliac bone[ 44 ]. In extreme cases of bone loss, the implant may be inserted in the sacrum.…”

“…Aseptic loosening rates reported for LUMiC® ranged between 0% and 10% in the short to medium term follow‐up 15,17,20,21 . No aseptic loosening was encountered in our series after a mean of 28 months of follow‐up, up to 5 years for the longest one.…”

“…This allows also for a timely surgical intervention, sparing the patients unnecessary surgical delays and hazards encountered with custom made implants. Four retrospective series in the recent medical literature reviewed the outcomes associated with the LUMiC® implant 15,17,20,21 . They analyzed mainly the mechanical and the infectious complications associated with its usage (Table 3).…”
