Abstract: Internal root resorption (IRR) is a rare pulp disease. Its etiology involves late pulpal inflammations and trauma, among others. IRR may also show some symptoms, and is usually detected by X-rays. However, its diagnosis is significantly improved by the use of cone beam computed tomography (CBCT). The objective of this case report was to account for the diagnosis and management of an internal root resorption without perforation. The patient, a 26-year-old male, went to the School of Dentistry at Universidad Andres Bello, Concepción, without having symptoms in the tooth 1.1. Anamnesis revealed the presence of previous symptoms. CBCT examination showed absence of bowl-shaped calcified dentin tissue on the inner walls of the root canal with apical lesion but without perforation of surrounding tissues. Endodontic treatment was performed using the following methods: irrigation of the root canal with 2% chlorhexidine (CHX) using a Max-i-probe cannula and simultaneous cavitation of the irrigant Then calcium hydroxide (CH) was applied as intracanal medication for a week and Schilder's technique for vertical compaction was used. The patient was checked after one week and then after six months. He did not have any symptoms. Early diagnosis using modern imaging equipment, appropriate use of ultrasound for chemomechanical debridement and thermoplastic filling techniques contribute to a more favorable prognosis of patients with internal root resorption.

ÖZETBu olgu bildiriminin amacı internal kök rezorbsiyonu gözlenen sağ üst lateral kesici dişte uygulanan tedavi protokolü ve olgunun üç yıllık takibinin sunulmasıdır. Otuz altı yaşındaki erkek hasta sağ üst çene lateral kesici dişinden kaynaklı ağrı şikâyeti ile kliniğimize başvurdu. Klinik ve radyografik değerlendirmelerde sağ üst lateral kesici dişte internal kök rezorbsiyonu varlığı tespit edildi. Giriş kavitesinin açılmasının ardından kök kanalları Hedström el eğeleri kullanılarak step back tekniği ile şekillendirildi ve seansın sonunda kalsiyum hidroksit patı kanala uygulandı. İki hafta sonra hastanın herhangi bir klinik şikâyetinin olmadığı gözlendi. İlgili dişin kök kanalı biyomekanik şekillendirmeyi takiben, gütaperka ve biyoseramik esaslı kök kanal patı ile lateral kompaksiyon tekniği kullanılarak dolduruldu. Tedaviyi takip eden 12. ayda ilgili dişten kaynaklı fistül varlığı ve orta şiddette ağrı tespit edildi; bu nedenle endodontik cerrahi tedavi uygulanmasına karar verildi. Tam kalınlıklı mukoperiostal flep kaldırılarak rezorbsiyon alanına ulaşım sağlandı, granülasyon dokusunun uzaklaştırılması ve defektin küretajını takiben Mineral Trioksit Agregat (MTA) ile rezorbsiyon alanı örtülendi. Üçüncü yılsonunda dişin asemptomatik olduğu ve periradiküler lezyonun önemli derecede iyileştiği gözlendi.Anahtar kelimeler: İnternal kök rezorbsiyonu, Mineral trioksit agregat, endodontik cerrahi ABSTRACT The aim of the present case report was to present the treatment approach and three years outcome of the treatment of a maxillary right lateral incisor with internal root resorption. Aim: This review aims to provide comprehensive information related to the principles of Cone beam computed tomography and its potential applications in the management of various endodontic conditions. Methodology:A thorough and extensive electronic literature search was conducted utilizing PubMed, for articles related to endodontic applications of CBCT published during the period between January 2005 and September 30, 2013. Search words such as 'principles of CBCT', 'endodontic applications of CBCT' were employed to obtain information. Only those articles dealing with the principles and various endodontic applications of CBCT are included in this review. Results:The search revealed 258 articles, 70 of which were found to be relevant to the scope of this review and were used in this review CBCT is a revolutionary and innovative procedure that has changed the paradigms in the management of various endodontic conditions. The information provided by this tool-three dimensional view of anatomic and pathologic structures, ability to provide details of root and canal anatomy, assessment of dento-alveolar trauma, assessment of root resorptions, etc. has contributed substantially to its extensive utilization in a short period of time.Clinical Significance: CBCT can be used in the management of a variety of conditions like dento-alveolar truma, root resorptions, early apical periodontitis, roots and canals with unusual anatomy, dental anomalies, etc. The effective dose of CBCT (focused field of view) varies from 5-38.3 μSv. The effective dose of intra-oral periapical radiographs and panoramic radiographs are ˂8.3 μSv and 9-26 μSv respectively. Thus CBCT has an effective dose in the similar range of magnitude as other dental radiographs, but, its three dimensional imaging capability and 100% sensitivity (1.0) and specificity (1.0) makes it an invaluable tool in the field of endodontics.
