Los carcinomas basocelulares (CBC) son los tumores malignos más frecuentes de la piel. Se localizan preferentemente en la cara (85%) y son las neoplasias más comunes de la región periocular, especialmente en el párpado inferior y en el canto interno. Afectan generalmente a sujetos de edad avanzada, ya que se asocian con exposición crónica a las radiaciones ultravioletas. En la última década se ha detectado un incremento de su incidencia y una tendencia a aparecer en edades más tempranas, posiblemente asociados al empleo de fototerapia y a la exposición solar 1 .Actualmente, la extirpación quirúrgica sigue considerán-dose la terapia más efectiva para los CBC debido a sus altas tasas de curación (95-98% según las series) 1 . Se ha observado que algunas células tumorales pueden presentarse fuera de los bordes de la lesión clínicamente visibles. Este es el motivo por el que numerosos autores aconsejan extirpar entre 3 y 5 mm de tejido sano alrededor del lecho tumoral para evitar recidivas 1,2 . Esta medida a nivel periocular puede implicar la extirpación de amplias áreas de los párpados o de las vías lagrimales, cuya reconstrucción puede causar, en mayor o menor medida, alteraciones estético-funcionales como cicatrices, triquiasis, retracción palpebral, ptosis, epífora cró-nica, entropion, ectropion, queratitis, úlceras corneales por exposición e incluso perforación ocular. La técnica quirúrgica que ha demostrado mayores tasas de curación es la cirugía micrográfica de Mohs. Sin embargo, su utilización no se ha generalizado en España; ya que requiere tiempos quirúrgicos prolongados y altos costes, reservándose para CBC agresivos como los localizados en el canto interno o de alta malignidad * Autor para correspondencia.Correo electrónico: egmvivax@yahoo.com (E. García Martín). histológica (infiltrantes, esclerosantes...) cuyas características hacen especialmente difícil la completa eliminación del tumor y su reconstrucción posterior 1,3 .La sociedad actual es más exigente con las expectativas que deposita en la Medicina, especialmente en relación a los efectos secundarios y a los resultados cosméticos y funcionales. La tendencia en el tratamiento oncológico de los diferentes tumores en el organismo es ser cada día más conservador, optando por asociar terapias coadyuvantes a la cirugía con el objetivo de extirpar la menor cantidad posible de tejido sano, reducir la morbilidad y los efectos secundarios de los tratamientos 1,4 . Esta tendencia al uso de terapias menos invasivas asociado con el hecho de que los CBC son tumores malignos de crecimiento localizado que metastatizan muy raramente, ha propiciado la búsqueda de tratamientos alternativos a la cirugía, como inmunomoduladores tópicos, terapia fotodinámica, crioterapia, radioterapia, retinoides y quimioterapia con agentes como el 5 fluorouracilo, el cisplatino, o la bleomicina [4][5][6] .
