Diabetes mellitus'lu hastaların tamamlayıcı ve alternatif tedavileri kullanma durumları ve diyabet tutumları ile ilişkisi-Kaynak İ. ve Polat Ü.Genel Tıp Derg 2017;27(2):56-64 56 Özet Amaç: Bu çalışma Diabetes Mellitus'lu hastaların tamamlayıcı ve alternatif tedavileri kullanma durumlarının diyabet tutumlarıyla iliş-kisini belirlemek amacıyla yapılmıştır. Gereç ve Bulgular:Hastalarda TAT kullanım oranının %48,1 olduğu ve kullananların tamamının bitkisel uygulamaları kullandığı belirlenmiş-tir. TAT kullanan ve kullanmayan hastaların diyabet tutum ölçeği puan ortalamaları arasındaki farkın anlamlı olmadığı saptanmıştır (p>0,05). TAT kullananlarda medeni durumun, TAT kullanmayanlarda ise hastalık ve tedavisi hakkında alınan eğitimi yeterli bulma durumunun diyabet tutumlarını etkilediği saptanmıştır (p<0,05). TAT kullanan hastaların ortalama HbA1c değerlerinin ( =%8,4 ± 2,2) kullanmayanlara göre (= =%7,8 ± 1,8) anlamlı olarak daha yüksek olduğu saptanmıştır (p<0,05). Sonuç:Bu çalışmada Diabetes Mellitus'lu hastaların TAT kullanımı ile diyabet tutumları arasında bir ilişki bulunmamıştır. Ancak TAT kullalan hastalarda HbA1c değerinin yüksek olmasından dolayı, kullanmayanlara göre glisemik kontrolün iyi olmadığı bulunmuştur. Bu nedenle sağlık profoyenelelrinin diyabetli hastalarda hastalığa karşı tutum ve TAT kullanımını belirlemelidir. Anahtar Kelimeler: Tamamlayıcı ve Alternatif Tedavi, Diyabet Tutum, Hemşirelik, Diabetes Mellitus AbstractObjective: This study was conducted to evaluate the relationship between use of complementary and alternative treatment and diabetes attitudes with diabetes mellitus patient. This is a descriptive and cross-sectional study. Materials and methods:285 diabetic patients who were admitted to the Endocrinology and Metabolic Disorders outpatient clinics at Ankara Numune Education and Research Hospital and who were coming for diabetes education were included in the study. "A questionnaire form" and the "Diabetes Attitude Scale " (DAS)were used to collect data. Results:The analysis of the study data revealed that 48.1% of the patients with diabetes used CAT and among patients that used the CAT, all used herbal remedies. It was showed no significant difference between DAS mean scores of diabetes patients using CAT and not using. Among CAT users marital status, among nonusers considering received education about the disease and its treatment sufficient affecting their attitude toward diabetes (p<0.05). HbA1c values of patients using CAT using (= = 8.4 ± 2.2%) were found significantly higher than those who use (= = 7.8 ± 1.8%). Conclusion:: Nurses should determine to determine the attitudes of the patients about the illnes and the use of complementary and alternative treatment for patients with diabetes mellitus.

The use of traditional medicines is common among patients with chronic illnesses and this practice might pose health risks. The use among Libyan patients with diabetes is unknown. Therefore, this study aimed to estimate the prevalence of traditional medicine use in the previous year among Libyans with type 2 diabetes and to examine the association between its use and sociodemographic and clinical characteristics of the patients. A cross-sectional study was conducted at a large diabetes centre in Tripoli. A self-reported questionnaire was used for data collection. Of the 523 respondents, 28.9% used traditional remedies. Sex was the only variable significantly associated with traditional medicine use; more women used traditional medicines (P = 0.01). A total of 77 traditional medicine items were reported to be used, of which herbs were the most common. The use of traditional medicine for diabetes is prevalent and some of the reported items could pose health risks. Health education programmes are suggested to raise the awareness of the health risks of this practice. RÉSUMÉ L'utilisation à la médecine traditionnelle est courante parmi les patients souffrant de maladies chroniques et cette pratique peut entraîner des risques pour la santé. On ignore dans quelle mesure les patients diabétiques libyens y ont recours. La présente étude avait pour objectif d'estimer la prévalence de l'utilisation de la médecine traditionnelle au cours de l'année précédente chez des patients libyens atteints de diabète de type 2, ainsi que d'examiner l'association entre cette utilisation et les caractéristiques socio-démographiques et cliniques des patients. Une étude transversale a été conduite dans un grand centre de traitement du diabète à Tripoli. Un questionnaire auto-déclaré a été utilisé pour la collecte de données. Sur les 523 répondants, 28,9 % prenaient des remèdes traditionnels. Le sexe des individus était la seule variable associée de façon significative à l'utilisation de la médecine traditionnelle, un plus grand nombre de femmes y ayant recours (p = 0,01). Un total de 77 produits de médecine traditionnelle ont été déclarés, les plus courants étant les plantes. L'utilisation de la médecine traditionnelle pour traiter le diabète est prévalente, et certains des produits rapportés peuvent constituer des risques pour la santé. Il est suggéré de mettre au point des programmes d'éducation sanitaire afin de sensibiliser la population aux risques que cette pratique représente pour la santé. ‫الثاين‬

Objectives: Ayurvedic oil-dripping treatment (Shirodhara) is often used for treating sleep problems. However, few properly designed studies have been conducted, and the quantitative effect of Shirodhara is unclear. This study sought to quantitatively evaluate the effect of sesame oil Shirodhara (SOS) against warm water Shirodhara (WWS) on improving sleep quality and quality of life (QOL) among persons reporting sleep problems. Methods: This randomized, single-blinded, crossover study recruited 20 participants. Each participant received seven 30-minute sessions within 2 weeks with either liquid. The washout period was at least 2 months. The Shirodhara procedure was conducted by a robotic oil-drip system. The outcomes were assessed by the Pittsburgh Sleep Quality Index (PSQI) for sleep quality, Epworth Sleepiness Scale (ESS) for daytime sleepiness, World Health Organization Quality of Life 26 (WHO-QOL26) for QOL, and a sleep monitor instrument for objective sleep measures. Changes between baseline and follow-up periods were compared between the two types of Shirodhara. Analysis was performed with generalized estimating equations. Results: Of 20 participants, 15 completed the study. SOS improved sleep quality, as measured by PSQI. The SOS score was 1.83 points lower (95% confidence interval [CI], -3.37 to -0.30) at 2-week follow-up and 1.73 points lower (95% CI, -3.84 to 0.38) than WWS at 6-week follow-up. Although marginally significant, SOS also improved QOL by 0.22 points at 2-week follow-up and 0.19 points at 6-week follow-up compared with WWS. After SOS, no beneficial effects were observed on daytime sleepiness or objective sleep measures.Conclusions: This pilot study demonstrated that SOS may be a safe potential treatment to improve sleep quality and QOL in persons with sleep problems.
