Introduction: Conscious sedation in dentistry becoming more popular in Malaysia nowadays. However, the knowledge ,exposure and practice of conscious sedation in dental setting has rarely been explored. Therefore, this study aimed to evaluate the knowledge awareness and exposure of Malaysian dentists about conscious sedation for dentistry and their association with sociodemographic profile. Materials and Methods: A cross-sectional study using an online questionnaire (Qualtrics@ Software) was conducted among Malaysian registered dentists. Questions on sociodemographic profile, knowledge, awareness, exposure and practice on conscious sedation were collected. Chi square test was used to analyse the associated factors for knowledge, exposure and practice. Result: A total of 166 respondents completed the survey, resulting in response rate of 43.1%. Majority agreed that conscious sedation is beneficial in allaying dental anxiety and knows at least 3 types of CS. More than half had been exposed in inhalation sedation (IS) with less than half had CS exposure during undergraduate (UG). However, only a quarter practicing CS in their dental practice. Discussion: Malaysian dentists were familiar with the indications of CS in dentistry which corroborates with other studies. Meanwhile, the exposure to CS during undergraduate study were dependant on the availability and the program structure. Conclusion: Majority of respondents know, but only some are practicing oral and inhalation sedation. Only a few of respondents know about other types of CS. Thus, there is a need for more exposure and training of CS during undergraduate to cater the need of patients with dental anxiety.

The challenges of sedating a child for dental procedures are multifactorial: The patient's age, health, temperament and emotional status, parental concerns, clinician philosophy on patient management, extent and quality of clinician training and experiences with sedation, state dental board regulation of sedation, issues of third-party coverage or parental reimbursement, knowledge of and adherence by clinicians to sedation guidelines, facility preparedness, and support staff experiences are but a few of many important considerations. To overcome these challenges, a pediatric dentist has to be at the hub of the preventive, operative and behavioral treatment plan.The fi rst section of this chapter is devoted to sedation practices performed primarily in the United States. The second section of this chapter describes sedation practices associated with the United Kingdom and Europe. The closing section will provide an overview of the state of sedation for pediatric dental procedures in South America, as an example of different solutions to sedation management in underdeveloped/developing countries.

Chloral hydrate is the oldest and most common sedative drug used in moderate sedation for pediatric dental patients. Hence, the purpose of this article is to review the safety and possible adverse events of this drug when used for pediatric dental treatment. A bibliographic search in PubMed, MEDLINE, Cochrane Library and KMbase, KISS, DBpia, KoreaMed, and RISS databases was performed. Using the keywords "dental sedation," "chloral hydrate," and "children or adolescent," 512 scientific articles were found. Subsequently, 183 studies were individually assessed for their suitability for inclusion in this literature review. Altogether, 24 studies were selected. They included 12 cases of death before, during, or after chloral hydrate sedation for dental treatment, majorly due to dosing error and use of multiple sedatives. Additionally, intraoperative adverse events were mostly respiratory problems such as hypoxia and apnea, but most events were temporary. After treatment, prolonged sedation, including excessive sleep and less activity were the most common postoperative adverse events, and even death cases were reported. Despite the wide acceptance of chloral hydrate as a sedative-hypnotic agent, the risk of adverse events and adequate dose should be of great concern when using it for pediatric dental sedation.
