The use of a double‐lumen central venous catheter for airway management in pediatric patients undergoing laryngeal papillomatosis surgery

Abstract: After establishing an adequate depth of anesthesia, a spontaneous ventilation anesthesia technique with insufflation of oxygen and volatile agent through a DLCVC is feasible in pediatric patients undergoing suspension laryngoscopic surgery for laryngeal papillomatosis.

“…A recent study described the use of a double‐lumen catheter inserted below the glottis through the laryngoscope, with anesthetic gases and oxygen insufflated through one port while the other is used to measure the end tidal CO 2 (ETCO 2 ) . Although the authors ( k.d.p., a.i.)…”

“…In a patient with wide glottic opening, controlled ventilation through a relatively small size endotracheal tube should be preferred and in a child with narrow glottic chink, DLCVC may be chosen. Two third of the patients (24 of 36) in the study had grade I obstruction only and probably not very narrow glottis chink. Hence, successful use of this device in large number of children with narrow glottic chink needs further evaluation.…”

“…SIR-It is difficult to assess patient's readiness for extubation in the operating room (OR) among the pediatric population (1). We seek to add to the available assessment tools by characterizing an airway reflex involving the genioglossus muscle as another potential marker of airway protection ability and extubation readiness.…”
