The use and abuse of oxytocin

Abstract: Key content Oxytocin is a well‐established drug with a long history of use. The generation of intrauterine pressure is a result of myometrial activity, but is attenuated by a softening cervix. The introduction of active management regimens have not changed the outcome of labour as we envisaged. Too many babies suffer asphyxia as a result of misuse of oxytocin. Some form of education programme must be developed to warn of the dangers of this very useful drug. Better training and education programmes must be i… Show more

“…Despite this titration, a common complication of its use is uterine tachysystole, more than five contractions in 10 minutes averaged over a period of 30 minutes (ACOG 2009). When the contractions are too frequent or too long, the relaxation period between them is too short, which may lead to lack of su icient oxygen supply for the child causing abnormal fetal heart rate (FHR), which may require immediate delivery by caesarean or instrumental delivery (Oláh 2015).…”

“…The same author stated that there is a gap between evidence-based medicine and clinical practice regarding the use of oxytocin augmentation during labor 231,307 . We would argue that our findings of a reduced use of oxytocin augmentation, reduced overall cesarean section rate, and a trend toward fewer children born with low pH in the umbilical cord should be emphasized in the ongoing debate concerning guidelines on the use of oxytocin augmentation 306 . Recently, a high level of lactate in the amniotic fluid was found to be predictive of who could benefit from oxytocin augmentation and of labor outcomes 308 .…”

“…We cannot draw conclusions about improvements in neonatal morbidity as this was not within the scope of Paper II. In addition, as these findings are less common, a larger study population or a different study design would be required to reveal these associations 306 .…”

“…However, as a recent retiree I found the update worthwhile. It has become very easy to “give an oxytocic” but the review of The use and abuse of oxytocin serves as a warning not to become casual about stimulating the uterus in labour.…”
