The Uptake of Nordic Hamstring Exercise Program for Injury Prevention in Major League Soccer and Its Barriers to Implementation in Practice

Abstract: Context: It is well demonstrated that Nordic hamstring (NH) program has poor compliance across European soccer clubs, but little is known about its implementation in Major League Soccer (MLS). The authors aimed to investigate MLS hamstring prevention practices and compliance with the NH program and understand the reasons for its inclusion or exclusion. Design: A cross-sectional electronic survey was completed by 24 of the 26 (92%) MLS top-division soccer clubs. Methods: The survey, based on the Reach, Efficacy… Show more

“…to optimise player engagement and address time barriers) 18 19. Alternatives to Nordics were also provided (eg, hamstring bridges) due to known barriers to use (eg, player soreness),55 and their effectiveness in women is unknown 54. Providing coaches with the option to move the strength component from the warm-up to the traditional ‘cool down’ period of training may optimise uptake and injury reduction effect 56.…”

“…Of the 10 players who accepted, four players declined adding weight beyond 15 kg and the remaining six did not want to go beyond 20 kg, despite not all of them reaching a plateau in the force measured. Fear of muscle soreness is one of the main challenges in obtaining adherence to injury prevention programmes from football players,27 28 and could also be a problem with Nordic hamstring strength testing with added weight.…”

“…20 The time duration of the prevention program and the presence of delayed onset muscle soreness are considered as major burdens that limit the compliance of professional soccer players. 6 The compliance of the players is crucial to improve the effectiveness of the prevention program. 4 In the current study, we found a moderate compliance rate (ie, 47%), which was close to the 50.1% recommended to ensure positive effects on hamstring injury incidence.…”

“…The major barrier that likely limits the compliance of professional soccer players is the presence of delayed onset muscle soreness associated with repeated intense eccentric contractions. [5][6][7] Manipulation of eccentric training volume and frequency has been proposed as an efficient strategy to overcome this barrier. 6 Indeed, reducing twice the number of eccentric contractions reduces the magnitude of delayed onset muscle soreness, 8 whereas a weekly volume between ∼20 and ∼70 repetitions does not affect the magnitude of the adaptations in eccentric strength and muscle architecture 9 .…”
