The transition from custom-made to standardized multibranched thoracoabdominal aortic stent grafts

Abstract: The substitution of SSG for CSG had no effect on the complexity of the procedure, the branch morphology, or the perioperative outcome. The availability of an off-the-shelf SSG will broaden the application of endovascular thoracoabdominal aortic aneurysm repair by eliminating manufacturing delays.

“…17 Custom-made aortic arch and branched arch devices have been employed successfully to treat arch pathologies, maintaining blood flow in supra-aortic vessels. 18 Also, in situ laser fenestration has been reported as a valid option to restore blood flow after LSA coverage. 6 However, it requires dedicated materials not always available, especially in the emergency setting.…”

“…Fifty devices (61.7%) were custom-made, with cuff position determined by the patient's anatomy (custom devices), and 31 (38.3%) were constructed using a consistent craniocaudal and circumferential cuff position (standard devices). 15,19 No spiral (directional) cuffs, cranially directed cuffs, radially directed cuffs, or fenestrations were used.…”

“…5 Self and balloon expandable stent grafts have been used to connect the AMB with the visceral vessels. They work as a bridge (bridging stent graft [BSG]) between the AMB and the target visceral vessel.…”
