The Transglabellar Subcranial Approach for Nasal Dermoids with Intracranial Extension

Abstract: The subcranial approach is an effective technique for the resection of nasal dermoids with intracranial extension. These lesions have traditionally been managed with lateral rhinotomy, midface degloving, or external rhinoplasty approaches combined with a frontal craniotomy. The subcranial approach offers excellent exposure, minimizes frontal lobe retraction, reduces the likelihood of cerebrospinal fluid leak, and provides for excellent cosmetic result. This approach was used in two cases with long-term follow-… Show more

“…Various techniques have been described, removing all or part of the frontal bones via a coronal 11,12 or subcranial approach. 13 The technique used in this series was a coronal flap combined with a bifrontal craniotomy to approach the intracranial component and a direct excision for the subcutaneous cyst or sinus. In one patient, debridement of necrotic, infected frontal bone was required to eliminate chronic osteomyelitis.…”

“…Given that these lesions, infection risk aside, are resected because of the cosmetic appearance then careful selection of approach is paramount. Midline vertical nasal incision, transverse nasal incision, paramedical incision, medial canthal incison and eyebrow incision, gullwing incision and bicoronal flap have all been advocated along with external rhinoplasty for nasal dermoids [4,[7][8][9]. All of these incisions leave a scar on the face.…”

“…Most publications on NDSC are case reports, small series of patients or reviews (5,10,(12)(13)(14)20) . Larger published series either combined ectodermal and neuroectodermal masses or they summarized di erent malformations under the label of NDSC (8,9,11) .…”

“…The approach for NDSC is still controversial. Several techniques have been suggested, including transfacial, open rhinoplasty and coronal approaches (5,(8)(9)(10)(11)(12)(13)(14) .…”
