BACKGROUND: Dengue is a disease caused by four distinct serotypes of dengue virus (DENV 1-4). DENV serotype differs from one another by 25–40% at the amino acid level. The detection of serotype is very important due to the fact that in secondary infection with heterologous serotype often leads to life threatening, dengue hemorrhagic fever (DHF)/dengue shock syndrome (DSS); likewise, an infection caused by two serotypes or more in one individual can contribute to the severity of infection. AIMS: The aims of the study were to detect the multiple dengue serotypes infection by quantitative reverse transcription polymerase chain reaction (Q-RT-PCR) and to determine the viral load in dengue infection. MATERIALS AND METHODS: This study applied the molecular examination for determining the serotype and viral load of DENV. The data were analyzed using Student’s t-test. RESULTS: A total of 119 samples, 91 samples showed positive dengue infection after amplification. The multiple dengue infection was found in 47 samples and 44 samples with single infection. There was a significant difference between the number of viral load DENV-2 and DENV-1 infection (p = 0.000). CONCLUSION: Two or more serotypes of dengue were found to infect a patient in West Sumatra. DENV-2 serotype was found predominantly in West Sumatra (n = 36, 39.56%) in patients with single infection. The molecular detection of dengue RNA by RT-PCR is a sensitive, rapid, and simple method. The RT-PCR method can detect the multiple dengue infection in clinical samples.

Introduction: Even after the discovery of vaccines for dengue fever, it remains a major public health problem in developing tropical and subtropical countries, especially in India. The dengue fever has a varied clinical spectrum ranging from a mild headache to severe organ impairment or bleeding, depending on the virulence and susceptibility of the individual. The current study investigated the clinical profile of laboratoryconfirmed dengue patients. A secondary objective was to find the role of haematological parameters in prognosis. Material and Methods: This was a retrospective study conducted in a tertiary care hospital at Madurai following a dengue fever outbreak between August 2017 to November 2017 among laboratory-confirmed dengue patients older than 16 years. All the relevant clinical and laboratory investigation details of the patients were obtained from the medical records of the hospital. Results: Fever was present in all the cases with an average duration of 4.86 ± 1.59 days followed by myalgia (57.39%), vomiting (46.96%), headache (30.43%) and abdominal pain (20%). Bleeding and hepatic complication was presented by 22.16% and 50.43%. The mean platelet recovery duration was 8.42 ± 1.74 days. A strong positive correlation between day of recovery (increasing trend of platelet) from the onset of fever and WBC recovery (r s value: 0.0.713, P value: <0.001) was observed. Conclusion: Dengue fever patients have varied clinical features varying from fever to severe hepatic complication. For assessing the prognosis of dengue fever, along with the continuous monitoring of clinical profile and platelet count, evaluation of WBC recovery is also recommended.

ABSTRAKInfeksi virus dengue (IVD) masih menjadi masalah di negara tropis dan sub tropis. Gejala IVD dan perubahan parameter hematologi seperti jumlah leukosit, trombosit dan kadar hematokrit. pada umumnya lebih berat pada pasien infeksi sekunder dengue virus (DENV) dengan serotipe yang berbeda. Risiko infeksi sekunder DENV meningkat seiring dengan bertambahnya usia. Hal ini menimbulkan landasan teori terhadap hipotesis bahwa semakin bertambah usia maka perubahan parameter hematologi semakin berat. Untuk menjawab hal tersebut dilakukan penelitian terhadap pasien IVD yang berusia dibawah 14 tahun dan memenuhi kriteria diagnostik WHO 2009. Diagnosis dikonfirmasi dengan IgG dan IgM antidengue. Hasil yang didapat adalah terdapat korelasi antara usia dengan kadar hematokrit dengan r = 0,248 dan p = 0,027 namun tidak ada korelasi antara usia dengan jumlah leukosit dengan r = 0,008 p = 0,943 dan trombosit dengan r -0,01 p = 0,929. Semakin bertambah usia maka semakin berat peningkatan kadar hematokrit pada anak penderita IVD. ABSTRACT Dengue virus infection (IVD) is still a problem in tropical and sub-tropical countries. Symptoms of IVDand changes in hematological parameters such as the number of leukocytes, platelets and hematocrit levels are generally more severe in secondary DENV infections with different serotypes. The risk of secondary DENV infection increases with age. This raises the theoretical basis of the hypothesis that the older the worsening changes in hematologic parameters. To address this, a study of IVD patients under 14 years old and meeting the 2009 WHO diagnostic criteria is done. The diagnosis is confirmed by IgG and IgM antidengue. The result are correlation between age with hematocrit level with r = 0,248 and p = 0,027 but no correlation between age with leukocyte count with r = 0,008 p = 0,943 and platelet with r -0,01 p = 0,929. It can be concluded that the more aged the more severe the increase in hematocrit levels in children with IVD.
