The Tolerability of Lactated Ringer's Solution and BSS Plus for Ocular Irrigation with and without the Morgan Therapeutic Lens

Abstract: Abstract. Objective: To evaluate two solutions, lactated Ringer's (LR) and a balanced salt solution (BSS Plus, Alcon Laboratories, Ft. Worth, TX), compared with normal saline solution (NSS), for ocular irrigation in healthy adult volunteers with and without the Morgan therapeutic lens (MTL). Methods: This was a prospective, double-blind, randomized study of healthy volunteers who were at least 18 years of age. Exclusion criteria included the use of analgesics within four hours of the study. A complete ophthalm… Show more

“…With the patient lying supine or reclined, an infusion set was used to irrigate the normal saline directly into the eyes after applying topical anesthetic drops and a kidney tray to collect the contaminated saline. Others have reported using a Morgan therapeutic lens, which is shown to be well tolerated, to aid the irrigation 4. Mydriatics such as atropine and homatropine and anti-inflammatory have been advocated in patients with anterior chamber inflammation, to reduce the risk of formation of posterior synechiae, as well as to improve patient comfort.…”

“…13 Irrigating the eye with solutions that have a pH less than 6.8 or greater than 8.2 can contribute to corneal pain and swelling. 14 One important consideration in ocular decontamination with NS is that most commercially available saline preparations have a pH range between 4.5 and 6.0.…”

“…There are some devices that facilitate washing, such as the Morgan Medi-FLOW ® Lens (MorTan Inc., Missaoula, Montana, USA), a sterile plastic device that resembles a contact lens attached to a tube which delivers a continuous and copious flow of solution to the injured eye, freeing medical staff to treat other injuries or to transport the patient without interruption and allowing simultaneous washing of both eyes in cases of bilateral involvement (Jones, Schoenleber, and Gillen 1998). Home irrigation devices can also be made with materials that can be found in all emergency services: the tube of a butterfly type of venous catheter (21 gauge) fenestrated with scissors and connected to an infusion system with a 500 cc saline or Ringer's lactate bottle (video 1 ) (Terzidou and Georgiadis 1997).…”
