The Third International Consensus Definitions for Sepsis and Septic Shock (Sepsis-3)

Abstract: and septic shock were last revised in 2001. Considerable advances have since been made into the pathobiology (changes in organ function, morphology, cell biology, biochemistry, immunology, and circulation), management, and epidemiology of sepsis, suggesting the need for reexamination.OBJECTIVE To evaluate and, as needed, update definitions for sepsis and septic shock.PROCESS A task force (n = 19) with expertise in sepsis pathobiology, clinical trials, and epidemiology was convened by the Society of Critical Ca… Show more

“…This reflects the third international consensus definition that sepsis is “life-threatening organ dysfunction caused by a dysregulated host response to infection”20 or what the previous sepsis definition conceptualized as “severe sepsis.”14 We did not require explicit sepsis codes because they were not introduced until 2002, after the start of our study.…”

“…Though the hs‐TnT assay has demonstrated greater sensitivity and negative predictive value in AMI, there are limited data comparing these 2 assays in the septic population 37. The development of the sepsis‐3 criteria could influence the interpretation of the results of this study 38. However, this cohort of severe sepsis and septic shock are less likely to be missed with either definition because they comprise the extreme spectrum of illness 39.…”

“…According to the Third International Consensus Definitions for Sepsis and Septic Shock (Sepsis‐3),5, 6, 7 sepsis is defined as “life‐threatening organ dysfunction caused by a dysregulated host response to infection.” The clinical criteria of sepsis are suspected or documented infection and an acute increase in the Sequential Organ Failure Assessment (SOFA) score of 2 points or more. Septic shock is defined as a subset of sepsis in which the underlying circulatory and cellular/metabolic abnormalities are profound enough to substantially increase mortality.…”

“…In out‐of‐hospital, emergency department, or general hospital ward settings, adult patients with suspected infection can be rapidly identified as being more likely to have poor outcomes typical of sepsis if they have at least 2 of the following clinical criteria that together constitute the quick SOFA (qSOFA): a respiratory rate of 22 breaths/min or higher, altered consciousness, and a systolic blood pressure of 100 mmHg or less 5, 6, 7. The qSOFA criteria can be used to prompt clinicians to further investigate for organ dysfunction, to initiate or escalate therapy as appropriate, and to consider referral for critical care.…”
