Objectives: To compare the performance of SEDIsystem^TM, a fully automated analyzer for the measurement of the erythrocyte sedimentation rate (ESR), with the manual Westergren method. Materials and Methods: Both methods were applied to 150 randomly selected subjects. The linear regression and Bland and Altman data analysis methods were used to measure the agreement between the automated and manual methods. Results: The regression analysis showed a good correlation between the two methods (r = 0.91). The Bland and Altman data analysis showed no systematic bias (95% confidence interval for mean difference); however, limits of agreement were between 11.52 and –37.88. This indicates that ESR values measured by the SEDIsystem may be 11.52 mm/h above or 37.88 mm/h below the reference method. A greater scatter of data was also observed with abnormally high (>25 mm/h) ESR results (mean of difference = –21.4 and limits of agreement = –45.2 and 2.26) compared with normal (<25 mm/h) readings (mean of difference = –3.9 and limits of agreement = –13.5 and 5.7). Conclusion: The Bland and Altman statistical analysis showed a wide degree of scatter between results obtained by the two ESR techniques that was not clearly demonstrated using the linear regression analysis. The automated system was found to underestimate ESR with the Bland and Altman statistical analysis, and therefore a correction factor is recommended.

This study was undertaken to examine the efficacy of a new erythrocyte sedimentation determination (ESR) method, MicroSed SR-system, and study the expected value of the ESR obtained with this method among the healthy Thai population aged 17 to 50 years old. This was a comparative study between standard Westergren and the MicroSed SR-system for ESR on 100 individual subjects. In addition, the expected range was calculated based on ESRs of 50 males and 50 females. From the study, the comparison-of-methods plot the comparison-of-methods plot MicroSed SR-system (Y) VS Westergren method (X) gave the least square linear regression equation of Y = 0.1.371 X + 0.842 (r = 0.904). The expected range of ESRs for males and females are 0 to 25.6 mm/hr and 0 to 37.5 mm/hr, respectively. Based on study results, the MicroSed SR-system method ESR result correlates well with the standard Westergren method. The derived expected range of ESR was similar to the standard references. The MicroSed SR-system can be used as an alternative method for ESR determination.

VesMATIC cube80™ is a newly developed automated method for measurement of erythrocyte sedimentation rate (ESR). AIMS: The aim of our study was to compare the ESR values by VesMATIC cube80™ against the modified Westergren method. MATERIALS AND METHODS: This cross-sectional study was conducted at clinical laboratory in Kasturba Hospital, Manipal on 162 patients. The samples taken were as per the recommendations charted out by International Council for Standardization in Hematology (ICSH) for comparing automated and manual Westergren method. STATISTICAL ANALYSIS USED: Bland and Altman statistical analysis was applied for evaluating VesMATIC cube80™ against the conventional Westergren method. RESULTS: The analysis revealed a low degree of agreement between the manual and automated methods especially for higher ESR values (25mm/hr.), i.e., mean difference -0.44 (95% limits of agreement, -41.23 to 40.37). This discrepancy which is of clinical significance was less evident for ESR values in the normal range <25 mm/hour (-1.73 mean of difference; -18.0 to 10 limits of agreement). CONCLUSIONS: The fully automated system VesMATIC cube80™ for ESR measurement tends to underestimate the manual ESR readings. Further studies and validation experiments would be required.
