The technique of bilateral selective vagotomy with the electrical stimulation test

Abstract: and in 6 (40 per cent) of the patients mentioned with early temporary diarrhoea. This type of diarrhoea occurred as frequently after selective vagotomy as after total vagotomy. The incidence of pyloric stenosis in the whole vagotomy series was 24 per cent and in the pyloroplasty group 23 per cent. The incidence of pyloric stenosis in the 79 pyloroplasty patients showing a noticeable delay in the second week was 28 per cent, suggesting that pyloric stenosis is not a predisposing factor. However, 5 out of the 10… Show more

“…Selective vagotomy and proximal gastric vagotomy are now being supplemented with either Burge's or Grassi's intraoperative tests in order to test the completeness of the denervation of the parietal cells (Burge and Frohn, 1969;Grassi, 1975). In some cases where these tests have confirmed the completeness of parietal cell denervation, recurrent ulcerations have been seen several months or years later (Wastell and Zervos, 1976;Storey et al, 1981).…”

“…The initial study was carried out in a group of three men and three women each of whom was undergoing the operation of selective vagotomy for chronic peptic ulceration (Burge and Frohn, 1969). Further clinical details of this group are set out in Table I, Cases 1-6.…”

“…A reliable peroperative test of vagal section would reduce the incidence of recurrent ulceration after vagotomy. The Burge test (Burge and Frohn, 1969) is non-reproducible and unquantifiable, and the Grassi (1971) test, although more accurate in localizing acid-secreting mucosa, is time-consuming and risks peritoneal contamination. We have designed an inexpensive, speedy, quantifiable and reproducible test based on Burge's stimulation principle.…”
