Previous studies have shown that Low intensity pulsed ultrasound(LIPUS) prevents polyethylene-debris-induced periprosthetic loosening in vivo, but the details of the mechanism by which it does so remain unclear. In this article, we used polyethylene debris induced RAW 264.7 cells as the in vitro model, and tested the effect of LIPUS on this model. Changes in the level of inflammatory cytokines, cell proliferation, and apoptosis were assessed. Gene overexpression and siRNA technique were applied, and the levels of expression of FBXL2, TRAF6, ERK, and related inflammatory cytokines were also measured. Results indicated that FBXL2-mediated TRAF6 ubiquitination and degradation also plays an important role in aseptic periprosthetic loosening process, and LIPUS prevents such loosening by strengthening this pathway.

Comparison of Platelet-Rich Plasma, Stromal Vascular Fraction (SVF), or SVF with an Injectable PLGA Nanofiber Scaffold for the Treatment of Osteochondral Injury in Dogs

Stromal vascular fraction (SVF) contains a small number of mesenchymal stem cells and has been used as a treatment for osteoarthritis and cartilage injury. Due to limited evidence of successful cartilage regeneration with injected stem cell therapies, there is interest in combining cellular therapies with injectable scaffolding materials to increase intra-articular residence times of stem cells and improve tissue regeneration. However, the safety of intra-articular injection of SVF combined with injectable scaffolds is unestablished. Also, it is unclear if SVF therapy is superior to more easily prepared biologics, such as platelet-rich plasma (PRP). The purpose of this study was to assess the safety of SVF when combined with an injectable poly(L-lactide-co-glycolide) nanofiber scaffold and to provide a comparison of SVF therapy to PRP. A total of 12 Beagles had osteochondral defects created in both medial femoral condyles and 4 dogs each were allocated to treatment groups of SVF ( = 4), SVF plus PLGA scaffolding ( = 4), or leukoreduced PRP ( = 4). One knee in each dog received treatment, and the contralateral knee was sham treated with saline. Dogs were assessed over a 6-month period, and outcome measures included functional, radiographic, biochemical, and histological assessments. PRP treatment resulted in improvements in lameness scores and objective kinetic assessments of function. There were no statistically significant improvements in function, cartilage biochemical composition, or histology for SVF-treated knees. The combination of SVF and the injectable PLGA scaffold had worse outcomes than other groups including sham treatment based upon functional, biochemical, and histological assessments, raising concerns over the safety of this scaffold for intra-articular injection.

Introdução: O alívio do quadro álgico e reestabelecimento da função articular na artroplastia de joelho caracterizam achados positivos nos pacientes submetidos à cirurgia, porém a poucos estudos regionais que investigam o impacto na qualidade de vida dos pacientes. Objetivo: Avaliar o impacto da cirurgia da ATJ nas atividades de vida diária do amazônida. Materiais e Métodos: estudo analítico descritivo, analisado quantitativamente. Participaram 15 indivíduos com ATJ unilateral, idade igual ou superior a 50 anos, de ambos os gêneros e que realizaram fisioterapia após a protetização. Aplicou-se a Escala de Atividades de Vida Diária (ADLS), referente à funcionalidade do joelho e o Questionário Nórdico Musculoesquelético (NMQ), que investigou a dor. Resultados: 53,3% dos entrevistados somaram pontos iguais ou acima de 65 na ADLS. Protetizados com 12 meses de prótese apresentaram melhor score de funcionalidade, classificados como bom 92,8 e excelente 97,1. No NMQ cerca de 87% dos entrevistados relataram ter referido sintomas (dor, desconforto ou dormência no joelho e, desses, somente para 73% tiveram a necessidade de se afastar de suas atividades usuais. Conclusão: Houve melhora na função do joelho protetizado. Apesar de alguns sintomas se fazerem presente após a ATJ, esses se apresentam em menor gravidade. Contudo, pode se confirmar que a ATJ atrelada a fisioterapia no pós operatório, contribuíram para a melhora da capacidade funcional e a redução dos sintomas que afetavam em maior intensidade a funcionalidade dos indivíduos dessa pesquisa. Abstract: Age is the most relevant single factor related to revision rate. Cross-linked PE has a statistical lower revision rate at 10 years compared to conventional PE and, in the OxZr group, the revision rate is 2 times lower than Co-Cr in the same group of age.
