The Surgical Treatment of Arnold-Chiari Malformation in Adults

Abstract: T HE ARNOLD-CHIARI malformation is a deformity of the hindbrain in which a tongue-like projection of the cerebellar tonsils protrudes through the foramen magnum down onto the cervical cord. The caudad portion of the 4th ventricle likewise is elongated downward. The upper cervical nerve roots pursue a cephalad direction, giving the impression that the brain stem has been drawn down through the foramen magnum.Tile malformation was named after Arnold 3 and Chiari 6 who independently described the anomaly in 1894 … Show more

“…2,26,28,55 Plugging of the obex 25 in an attempt to block presumed CSF flow into the central canal has not worked well, carries a manipulation-related complication risk, 70 and has therefore been largely abandoned. In the present series the area of the obex was carefully avoided, and there were no complications associated with lower brainstem function.…”

“…10 He proposed that the fundamental mechanism underlying the origin of the Chiari malformation is delayed opening of the membrane covering the outlet of the fourth ventricle during fetal development and transient obstructive hydrocephalus with resulting for aminal herniation of the hindbrain and cerebellar tonsils. One of the difficulties with this potential mechanism is that it would not explain the small posterior fossa that has been described by several investigators in many patients with a Chiari I malformation since Gardner’s reports.…”
