Résumé -Introduction :Le bavage est secondaire à une altération de la musculature oro-faciale responsable d'une accumulation excessive de la salive au niveau du plancher antérieur qui entraîne sa fuite hors de la bouche. Il n'est pas généralement accompagné d'une hypersalivation. Matériel et méthodes : Sept patients porteurs d'un handicap psychomoteur présentant un bavage chronique et sévère, dont les thérapeutiques médicales ont abouti à l'échec, étaient candidats à une dérivation bilatérale des canaux de Wharton. Ce procédé était efficace sans aucune morbidité. Discussion : La prise en charge du bavage est multidisciplinaire. La chirurgie est indiquée après échec du traitement médical. Nous décrivons la technique de dérivation des canaux de Wharton et nous faisons l'état de l'art actuel sur les différents moyens chirurgicaux en précisant leurs avantages et leurs inconvénients. Abstract -Submandibular duct relocation in the treatment of drooling. Surgical technique. Introduction:Drooling is secondary to impaired orofacial muscles responsible for an excessive accumulation of saliva in the anterior floor that leads to its escape from the mouth. It is not usually accompanied by hypersalivation. Materials and methods: Seven patients with a psychomotor handicap with chronic and severe drooling, for which medical treatments were not successful, underwent bilateral submandibular duct relocation. This process was efficient without any morbidity. Discussion: The management of drooling is multidisciplinary. Surgery is indicated after failure of medical treatment. We describe the relocation of the submandibular duct and report the current state of the art of different surgical methods, describing their advantages and disadvantages.

Sialorrhea or drooling represents quite a common problem in patients with amyotrophic lateral sclerosis (ALS). In this review, we describe the possible treatments for this issue. Current medical management is not always effective: anticholinergic drugs (atropine, glycopyrrolate, amitriptyline, hyoscyamine, and transdermal scopolamine) are often used, but there is very little evidence of their effectiveness in patients with ALS. More invasive treatments, such as botulinum toxin injections and/or radiation therapy in the salivary glands, can be considered when anticholinergic drugs are not effective. In this review, we also explore the possible surgical options for treatment of sialorrhea. Although no specific studies have been conducted on patients with ALS, surgical therapies might represent a valid option for treatment of sialorrhea since there is no tachyphylaxis or need for repeated therapeutic sessions.
