The spectrum of progressive multifocal leukoencephalopathy: a practical approach

Abstract: John Cunningham virus (JCV) infection of the central nervous system causes progressive multifocal leukoencephalopathy (PML) in patients with systemic immunosuppression. With the increased application of modern immunotherapy and biologics in various immune‐mediated disorders, the PML risk spectrum has changed. Thus, new tools and strategies for risk assessment and stratification in drug‐associated PML such as the JCV antibody indices have been introduced. Imaging studies have highlighted atypical presentations … Show more

“…All of our patients were tested in-house using the Euroimmune commercial biochip immunofluorescence cell-based assay. Although there is a possibility of false-positive results in some of the cases, the risk is low as the Euroimmune cell-based assay has an 82.1% positive predictive value (18,19).…”

“…Immunotherapy might be another potential trigger. Treatment with immunotherapy has been linked to progressive multifocal leukoencephalopathy (PML) caused by John Cunningham virus . Expert opinion from neurology and radiology suggest that the patient's case lacked the clinical and radiological hallmarks for PML .…”

“…We read with interest the article 'The spectrum of progressive multifocal leukoencephalopathy: a practical approach' by Bartsch et al [1]; the authors also mentioned individuals affected by sarcoidosis among the patient groups where increased vigilance for progressive multifocal leukoencephalopathy (PML) is needed. Indeed, in this regard, here we report on a sarcoidosis patient who developed PML presenting with an isolated pseudobulbar palsy.…”
